【摘 要】 目的 探讨局灶性肿块样肺炎临床诊断与鉴别。 方法 回顾性分析本院1998～2004年间20例局灶性肿块样肺炎患者的临床表现、影像学特征及治疗结果以探讨局灶性肿块样肺炎临床诊断与鉴别。 结果 20例局灶性肿块样肺炎，其中包括5例早期误诊为肺癌的患者，抗感染治疗2周后，19例临床症状基本缓解，胸片显示病灶有不同程度的吸收，随访到第4～6周，胸片显示病灶完全吸收。1例在抗感染治疗2周后临床症状无明显缓解，胸片显示病灶吸收不佳，建议外科手术，病理检查诊断为慢性炎性。 结论 局灶性肿块样肺炎在临床症状和早期影像学表现与其他多种肺部疾病相似，不易鉴别，但结合痰培养，纤支镜检查及抗感染治疗前后临床表现和影像学变化特点的对比可提高诊断率。
