严重髋臼骨折早期全髋置换术效果观察

摘要： 目的 探讨早期全髋置换术治疗严重髋臼骨折的疗效。方法 对17例髋臼骨折移位施行早期全髋置换术，关节置换手术时间平均8天(5~21天)。平均随访时间2.1年(1~6年)。结果 随访根据Harris评分，平均为82分(69~100分)。15例疗效为优良；1例发生异位骨化，随访时没有松动；1例发生髋臼杯严重中央移位。结论 对严重的髋臼骨折常规切开复位内固定术难以取得良好疗效，施行早期全髋置换术是有效的治疗方式，可获得无痛、活动自如的髋关节。该手术最好由对于髋臼创伤和全髋置换术经验丰富的骨科医生施行。关键词：髋臼骨折；置换术；髋
Early total hip replacement without cement after severe acetabular fractures

Abstract: Objective To investigate effect of total hip replacement for severe fracture of acetabulum.Methods Total hip replacement was carried out in 17 cases of severe fracture of acetabulum from 1997 ~2003.The 17 cases were followed up averaged 2.1 years.The period from fracture to operation averaged 8 days(5~21d).The operative indication were： splintered fracture inside the joint,damage of all the cartilage on the articular surface,compact of the femoral head,the sag which was refered to more than 40% of the articular surface of the acetabular bone.Results The last follow�瘙楌up gained 82(69~100) points according to the Harris score.The effects of the 15 cases were good.The heterotopic ossification took place in 1 cases.Until the last followup there was not a case of lossening of artificial joint.One case of displacement of the central acetabular cup was found.Conclusion For the patients with severe fracture of acetabulum total hip replacement is an effective method. ......
