腹腔镜胆囊切除术的麻醉与手术问题探讨

【摘要】 目的：探讨腹腔镜胆囊切除术(LC)的麻醉与手术问题。方法：回顾分析375例患者的临床资料，其中硬膜外麻醉332例(88.5%)、硬膜外+气管插管麻醉27例(7.2%)及气管插管静脉复合麻醉16例(4.3%)。结果：3种麻醉方法均满足LC的要求。其中，中转开腹1例(0.3%)，发生并发症6例(1.6%)，无死亡病例，均痊愈出院。结论：3种麻醉方法的恰当选用，更能发挥各自优势及处理好病情与安危问题。ASA Ⅰ~Ⅱ级患者选择硬膜外麻醉安全可行且经济。【关键词】 胆囊切除术，腹腔镜；麻醉

【Abstract】 Objective:To discuss the anaesthetic methods of laparoscopic cholecystectomy(LC).Methods:The clinical data of 375 patients were retrospectively analyzed.Among them,332 patients(88.5%)were performed LC with epidural anaesthesia,27 patients(7.2%)with epidural plus tracheal intubation anaesthesia,and 16 cases(4.3%)with tracheal intubation intravenous anaesthesia.Results:The effects of the three anaesthetic methods were all satisfying,and all the cases recovered smoothly with one case(0.3%)converted to laparotomy,six cases(1.6%)had complications and no dead cases.Conclusions:To choose the anaesthetic of different methods and resolve the relationship between pathogenetic condition and safety.The epidural anaesthesia is safe and economic for patients with the cardiac function ASA ⅠⅡ. ......
