阿奇霉素治疗早期梅毒患者血清学变化的分析

摘要:目的 观察阿奇霉素在早期梅毒治疗中的疗效并探讨免疫印迹法(WB)在梅毒血清学诊断中的意义。方法 对确诊为早期梅毒的26例患者采用阿奇霉素500 mg/d，1次/d，连续10 d空腹口服治疗。治疗后3个月、6个月、1年、2年随访并做快速血浆反应素(RPR)、梅毒颗粒凝集实验(TPPA)、WB血清学检查。结果 治疗3个月后26例临床症状明显消失或消退，RPR 17例(65.4%)阴转，WB 3例(11.5%)阴转； 6个月后RPR 25例(96.2%)阴转，WB 3例(11.5%)阴转； 1年后RPR 26例(100%)阴转，WB 4例(15.4%)阴转；2年后 RPR 26例(100%)阴转，WB 10例(38.6%)阴转。结论 RPR在短时间内转阴率高，阿奇霉素治疗早期梅毒疗效肯定。免疫印迹法在梅毒的早期诊断中具有高度特异性和敏感性，可以作为早期梅毒的确诊方法。关键词:早期梅毒；阿奇霉素；免疫印迹法；特异性；敏感性
Analysis of the serum changes in the patients suffering from syphilis in the early stage after taking
azithromycin

Abstract: Objective To observe the clinical and serology effect of curing early stage syphilis after taking azithromycin,and evaluate western blotting assay (WB) in the serological diagnosis of human syphilitic infections. Methods As to the observed 26 syphilis cases took 500 mg of azithromycin for 10 days in a row. After 3,6 m,and 1 and 2 years,RPR,TPPA,WB were used to detect 26 serum specimens. Results Positive of RPR transformed negative in 17 cases out of the 26 cases after 3 m,and the rate is 65.4%. There were three cases that positive of WB transformed negative (11.5%). Positive of RPR transformed negative in 25 cases after 6 m,the rate is 96.2%,but the rate of WB was 15.4%. Positive RPR transformed negative in 26 cases out of the 26 cases after one and two years,the rate is 100%. The rate of WB was 15.4% and 38.6%. After two years of observation,no diseases were found and no positive of RPR again. Conclusion Instead of penicillin,azithromycin has high curative effect on early-stage syphilis,which can transform positive of RPR to negative quickly and shorten the time of treatment.. Western blot assay is sensitive and specific as a treponemal confirmatory test. ......
