     [摘要]目的探讨腹腔镜子宫切除术对患者单核细胞免疫功能的影响。方法择期实施子宫切除术患者20例，按手术方式分腹腔镜(LH)组和开放手术(AH)组各10例，分别于术前及术后第1、3、5天取静脉血检测单核细胞白细胞抗原(HLADR)、白细胞介素6(IL6)mRNA表达量及血清C反应蛋白(CRP)水平。结果 AH组患者术后第1天及第3天HLADR表达降低，IL6mRNA及CRP升高，直到术后第5天才恢复；LH组患者上述值无明显变化。结论手术创伤大小可影响单核细胞免疫功能，腹腔镜子宫切除术有助于稳定单核细胞免疫功能，为患者术后迅速恢复提供了良好的病理生理基础。

    Abstract：Objective To investigate the effect of laparoscopic hysterectomy on monocyte immunity. Methods Twenty hysterectomy patients were divided into two groups,laparoscopic hysterectomy(LH) and abdominal hysterectomy(AH) groups. Monocyte immune activity of human leukocyte antigen (HLADR), interleukin6(IL6)mRNA and Creactive protein(CRP) were evaluated before surgery and 1,3 and 5 days after operation. Results One and three days after surgery,increase in IL6mRNA and CRP, decrease in HLADR were detected in AH group. Their levels in AH group were restored nomal in 5 days. Compared with the baselines,all values remained unvariable within 3 days after surgery in LH group. Conclusion Laparoscopy is benefit for the monocyte immunity function suppressed by surgery. ......
