椎管内神经鞘瘤MR表现与病理联系

【摘要】 目的 探讨椎管内神经鞘瘤MRI表现特征。方法 回顾性分析22例椎管内髓外硬膜下神经鞘瘤的MR表现和病理对照。结果 椎管内神经鞘瘤T1WI低或等混合信号，T2WI高或高低混合信号，增强绝大多数肿瘤周围强化，伴或不伴有肿瘤内分隔。病理上，神经鞘瘤MRI上不均匀信号区，可以代表囊变、出血、胶原沉积、纤维化或钙化。结论 椎管内髓外硬膜下神经鞘瘤的MRI特征，有助于椎管肿瘤的鉴别诊断，尤其是伴有瘤内分隔性周围强化，应提示神经鞘瘤。【关键词】 磁共振成像；神经鞘瘤；病理学
Intradural schwannomas of the spine:MRI findings with pathological correlation

【Abstract】 Objective To evaluate the MRI appearances and correlation of pathological findings of intradural schwannomas.Methods MRI appearances of 22 cases of intradural schwannomas confirmed surgically and pathologically were retrospectively reviewed and analyzed.Results Intradural schwannomas were hypodensy or mixed hypo-isodensy on T1WI and were hyper-densy or mixed hyper-hypodensy on T2WI.Compared with the signal intensity of the spinal cord.The most lesions showed peripheral contrast enhancement with or without separations in the tumors.Pathologically,focal inhomogeneities of the intradural schwannomas and meningiomas on MR images represented cyst formation,hemorrhage,collagen deposition,fibrosis and calcification respectively.Conclusion Recognition of the MRI characteristics of intradural extramedullary schwannomas may be helpful in the differential diagnosis of the spinal tumors.In particular,peripheral contrast enhancement with separations of an intradural extramedullaty tumor on MRI will suggest the diagnosis of schwannoma. ......
