【摘要】 目的 探讨如何降低早产发生率及早产儿病死率。方法 对286例早产的主要病因，比较不同孕周、体重及不同促胎肺成熟治疗时间对早产儿并发症、死亡率的影响。结果 胎膜早破、双胎是自然早产的主要病因，胎儿宫内窘迫、妊娠高血压综合征、胎盘出血是引起干预性早产的主要病因；胎龄越小早产儿病死率越高；早期使用糖皮质激素可降低新生儿呼吸窘迫综合征(NRDS)的发生率。结论 不同病因的早产早发现、早治疗、延长孕周、适时宫内治疗，可提高早产儿的成活率。正确认识早产原因，及时做好早产的防治措施，是降低早产儿发病率及死亡率的根本方法。
