末节断指再植手术方法的选择及应用

【摘要】 目的 探讨末节不同节段断指再植的手术方法的选择及应用。方法 回顾我科自2003年3月以来，有完整资料记载的不同离断节段的末节断指共85例(134指)，分别采用3种方法再植：(1)常规吻合动静脉；(2)吻合双侧指动脉并近端单侧指动脉结扎；(3)吻合末节指动脉弓中央支并甲床放血。结果 134指术后129指成活，成活率96.3%。按朱盛修断指再植的功能评价，优良率98.1%。结论 针对末节断指的不同离断节段，只有灵活选择相适应的再植手术方法，扩大末节再植的手术适应范围，才能提高再植成活率，降低残肢率，从而恢复肢体良好的外观及功能。【关键词】 再植术；显微外科手术

【Abstract】 Objective To probe into the selection and application of the operation methods for paratelum different segments replantation amputated finger.Methods Retrospect 85 cases,134 paratelum different segment replantation amputated fingers with complete record since 2003.3.have been replanted in our department operative application:(1)AtV anastomos commonly;(2)anastomose both sides artery and deligate proximal end artery of fingers;(3)anastomose paratelum arterial orcades central one and nail bed bloodletting.Results 128 fingers alive,survival rate 96.3%;refer to Zhu Shengxius function standard of replantation of amputated finger,fineness rate 98.1%.Conclusion To aim directly at paratelum different segment amputated finger.Only agilitily select corresponding replantations methods,enlarge paratelum replantations operative scale,the survival rate of replantation will be elevated,debase disability. ......
