桥本病外科治疗32例的临床分析

     【摘要】 目的探讨桥本病的外科治疗时机和方法。方法回顾1996～2005年手术治疗的桥本病32例患者的临床资料，总结外科治疗的适应证、方法和结果。结果 32例病人术前表现为：甲状腺肿块23例、甲状腺重度肿大4例、甲亢3例、甲状腺肿大伴疼痛2例，其中细针穿刺提示癌变2例。所有病人术后均经病理证实为慢性淋巴细胞性甲状腺炎，其中4例合并甲状腺癌。结论对桥本病合并孤立的甲状腺肿块有恶性可能、甲状腺重度肿大产生压迫症状、合并疼痛，甲状腺素治疗无效的桥本病人应积极手术治疗。术中根据冰冻病理切片结果决定手术方式，术后病人应给予甲状腺素治疗。

    The surgical treatment of Hashimotos disease:a retrospective study of 32 cases

    【Abstract】 Objective To discuss surgical indication and management of Hashimotos disease.Methods We collected thirtytwo cases of Hashimoto's disease treated by surgical procedure during 1996～2005,and concluded their indications,management and effectiveness.Results The primary manifestations of the 32 cases before operation included:thyroid nodules(n=23),thyroid seriously enlarged (n=4),hyperthyroidism(n=3),thyroid enlarged with pain(n=2).Two cases were diagnosed as thyroid cancer by fineneedle aspiration.All the cases were confirmed to be chronic lymphocytic thyroiditis by pathological examination,among them four cases were accompanied by thyroid cancer(12.5%).Conclusion We concluded the surgical indication:carcinoma under suspicion ......
