【摘要】 目的 探讨脂肪肝与糖代谢异常之间的关系。方法 (1)将234例受检者按照B超诊断结果分为脂肪肝组和非脂肪肝组，检测血脂、血糖和胰岛素含量，计算胰岛素抵抗指数(HOMA-IR)；(2)将脂肪肝组中无糖代谢异常的受检者分为治疗组(服用贝特类降脂药物)和非治疗组，连续观察3年，检测其血脂、血糖和胰岛素含量，计算HOMA-IR。结果 脂肪肝组糖代谢异常的发生率及血甘油三酯(TG)、血糖、胰岛素含量及HOMA-IR均显著高于非脂肪肝组(P<0.05～0.001)；脂肪肝治疗组糖代谢异常的发生率及血TG、血糖、胰岛素含量及HOMA-IR较治疗前显著降低(P<0.05～0.001)，非治疗组血TG、血糖、胰岛素含量及HOMA-IR较3年前显著升高(P<0.05～0.001)。结论 脂肪肝与糖代谢异常关系密切，积极治疗脂肪肝有助于推迟或防止糖代谢异常的发生。
