磁刺激运动诱发电位与间隙内压监测对足筋膜间隙 综合征诊断的对照研究

     【摘要】 目的 探讨磁刺激运动诱发电位对足筋膜间隙综合征的诊断价值。方法 1996年6月~2004年10月,对241例足部损伤患者经双侧腘窝磁刺激坐骨神经。以最大输出量的70%刺激。同心圆针电极插入足部各筋膜间隙内相应的肌肉中,记录各自的运动诱发电位的潜伏期及波幅变化。同时采用Whiteside法测压装置监测足部各筋膜间隙内压的变化。结果 26例主诉伤足无法忍受的剧痛进行性加重,并伴有明显伤足肿胀,相应肌肉的MEP潜伏期明显延长,当潜伏期为健侧的(1.9±0.22)倍时,筋膜间隙内压均>30mmHg,确诊为足筋膜间隙综合征。结论 通过磁刺激运动诱发电位与间隙内压监测对足筋膜间隙综合征诊断的对照研究发现,磁刺激MEP作为诊断足筋膜间隙综合征的客观指标具有重要的价值。

    Comparisive study on the magnetic stimulation motor evoked potential and the interstitial pressure measure for the diagnosis of foot compartment syndrome

    【Abstract】 Objective To explore the diagnostic value of magnetic stimulation motor evoked potential (MEP) in foot compartment syndromes.Methods Between June 1996 and October 2004,the sciatic nerves in 241 patients with feet injuries were stimulated magnetically at the upper back knee.The stimulating intensity was maximal 70%.The electordes were inserted into the muscle which were included by itself foot department.The impedance of the electrodes were less than 5kΩ.The filtration wave were 20~200Hz.The change of latency and amplitude of MEP were recorded.At the same time,the clinical presentation was noticed and the interstitial pressure was measured with Whiteside's method.Results 26 patients complained to have more and more unendurable pain of foot accompanied with swelling.The latency of MEP for the muscle included in this department prolonged significantly.When the latency was as long as (1.9±0.22) times of normal leg,the foot interstitial pressure was more than 30mmHg.These cases were diagnosed as foot department syndromes.Conclusion It easy to find that magnetic stimulation MEP has an important value in the diagnosis of foot compartment syndromes by comparisive study on the magnetic stimulation MEP and the interstitial pressure measured for diagnosis of foot compartment syndromes. ......
