乳腺小肿块的超声影像与病理对照

【摘要】 目的 分析乳腺小肿块的声像图表现，并与病理对照，发现其异同点，以期提高诊断符合率。方法 取172例患者239个乳腺小肿块的声像图表现与病理对照分析，并从患者的年龄、乳腺小肿块的形态、方位、内部回声、后壁回声、包膜、边缘、边界等多方面来分析。结果 239个乳腺小肿块发病最高的3处依次为位于乳腺外侧、乳腺上方及外上象限。超声检查与病理诊断对比，符合率为47.7%，良性为49.3%，恶性为34.6%。乳腺病52个，导管扩张27个，囊肿19个，间质胶原化24个，浆液性乳腺炎1个，肉芽肿性乳腺炎1个，叶间脂肪3个，纤维腺瘤样增生26个，纤维腺瘤54个，导管内乳头状瘤6个，导管内乳头状癌2个，浸润性导管癌19个，导管内癌4个，髓样癌1个。结论 超声诊断中须抓住乳腺小肿块声像图改变与大体病理改变之间的规律性改变、乳腺小肿块良恶性的超声鉴别诊断要点以及减少或避免误诊的措施，从而进一步提高诊断符合率。【关键词】 乳腺小肿块；超声；病理
Comparison of pathologic results and ultrasonographic findings in breast small tumour

【Abstract】 Objective To analyze the samilarity and difference of ultrasonographic image for breast small tumour and the result of pathology with aim to improve diagnostic lever.Methods The author compared the results of pathology with the ultrasonographic images of 172 patients surferred from 239 breast small tumours with regard to patients age 、the tumours form、position、inner echo、behind echo、film、edge、 border.Results The breasts lateral and upper parts were in turn the most common foci of 239 breat small tumours with the rate of benign 49.3% and malignant 34.6%.The diagnostic accordance was 47.7%.The results of pathology diagnosis were as the following:52 cases were adenosis,27 caces were duct ectasia,19 cases were cyst,24 cases were mesenchymal fibrosis,1 case was plasma cell mastits,1 case was granulomatousl mastits,3 cases were fat,26 cases were fibrous disease,24 cases were fibroadenoma,6 cases were intraductal papilloma,2 cases were intraductal carcinoma,19 cases were infiltration of ductal carcinoma,4 cases were intraductal carcinoma,1 case was medullary carcinoma.Conclusion To improve diagnostic accuracy,the ultrasound diagnosis of the breast small tumour must grasp what followlls:(1)the regular changes of ultrasonography images and pathology for breat small tumour;(2)the differentiation diagnosis between benign and malignant of breat small tumour;(3)the methods which can reduce or avoid wrong diagnosis. ......
