摘要: 目的:探讨移床运动追踪磁共振血管成像(MRA)在下肢动脉疾病中的应用价值以及移床技术的优势。方法:对36例临床疑有下肢动脉疾病的患者行移床运动追踪三维动态增强MRA检查,其中急性下肢动脉闭塞症5例检查后行急诊手术,慢性下肢动脉闭塞31例,其中10例行数字减影(DSA)检查。10例可评价病例以DSA作为金标准进行效果评价。结果:36例病人均获得清晰图像,满意显示了下肢动脉的正常结构和疾病分布情况。将每条下肢分为14段血管节段观察,15例可评价病例共观察420段动脉节段,显示可比动脉节段102 段。与DSA比较,两者诊断符合率为 93.14%(95/102),MRA诊断的敏感度为100%,特异度为100%。结论:移床运动追踪MRA与DSA具有很好的一致性,能全面、准确地评价下肢动脉病变的程度和范围,是一种可靠、无创、有应用价值的检查方法。关键词: 磁共振血管成像;移床运动追踪技术;增强;下肢动脉
Clinical application of 3D contrastenhanced movingbed MRA in
arterial diseases of lower extremities

Abstract: Objective: To study the clinical value of 3D contrastenhanced movingbed MRA in the diagnosis of lower extremity artery diseases and to explore the advantages of movingbed technique. Methods: Thirtysix patients suspected of lower extremity arterial diseases were undergone 3D contrastenhanced movingbed MRA. Among these patients, 5 cases who suffered from acute occlusion of lower extremity artery were detected by emergency surgery. DSA was carried out in 10 cases who endured chronic occlusive diseases of lower extremity artery. The 15 cases diagnosis based on MRA was compared with DSA which is so called the “gold standard”. Results: All 36 cases got clear pictures which could directly and wholely show normal structure of artery and conditions of disease distribution. Every leg was divided into 14 vascular segments, 420 arterial segments of 15 cases were demonstrated. Taken DSA and operation as standard, 102 arterial segments were evaluated. The rate of accordance was 93.14% (95/102), MR angiography achieved sensitivity and specificity of 100% and 100%, respectively. Conclution: Compared with DSA, there is excellent correlation among 3D contrastenhanced movingbed MRA and DSA. MRI Mobitrack technique can accurately and comprehensively assess occlusive disease of arteries of lower extremity. It is a noninvasive, reliable and potential new technique. ......
