【摘要】 目的 通过多层螺旋CT对胰腺癌患者进行三维血管重建，了解肿瘤对胰腺周围血管的侵犯情况，对肿瘤的可切除性进行前瞻性评估，评价多排螺旋CT血管重建技术在胰腺癌可切除性术评估中的价值。方法 对胰腺癌患者进行术前CT检查和三维血管重建，将图像进行容积重建(VR)、多平面重建(MPR)、最大密度投影(MIP)等处理，显示胰腺癌对周围血管的侵犯情况，通过手术结果进行分析对照。结果 接受手术治疗患者78例。术前综合评估不能切除的40例中，36例手术无法切除，准确度为90%。术前综合评估能切除的38例中，实际切除29例，准确度为76%。结论 术前用多层螺旋CT对胰腺癌患者进行三维血管重建，可提供可靠的肿瘤和周围血管侵犯情况的影像学资料，对胰腺癌术前可切除性评估有重要的临床意义。

    Pancreatic carcinoma:preoperative resectability assessment by using multislice CT image reconstruction

    【Abstract】 Objective To investigate surgical removablity of pancreatic carcinoma through multislice spiral CT (CMSCT) with 3-dimentional vessel reconstruction and assess the scanning value.Methods Multislice CT scanning and 3D vascular image reconstruction were performed in patients with pancreatic carcinoma pre-operatively through VR,MPR and MIP to see the vascular invasiveness.The CT image findings was evaluated by comparing with surgical findings.Results Fourty cases regarded as irremovable by CT pre-operatively,of which 36 cases were proved in precedure,indicating the accuracy of CT was 90%.Of 38 cases regarded as removable pre-operatively,29 cases were treated surgically accounting for 76%.Conclusion Pre-operative multislice CT image evaluation is of great use for prediction of surgical removability in patients with pancreatic carcinoma pre-operatively. ......
