[摘 要] 目的：探讨糖尿病并泌尿系感染的临床特点及细菌耐药情况。方法：对我院内分泌科自1995年10月至2004年10月9 a间住院收治320例糖尿病并泌尿系感染患者的临床资料进行回顾性分析。结果：引起感染的病原体以革兰阴性杆菌为主，占90.2%，大肠埃希菌最常见占65%,其次为变形杆菌、副大肠杆菌、阴沟肠杆菌、产气杆菌等。影响感染的因素为：女性显著高于男性(P<0.01)；老年显著高于青壮年(P<0.01)；病程长者高于病程短者；并发酮症酸中毒、高渗性昏迷者高于一般病者；高血糖者高于低血糖者(P<0.01)；有膀胱病变者高于无膀胱病变者；无症状性菌尿高于有症状者。药敏试验结果显示，亚胺培南、头孢哌酮舒巴坦，沙星类的细菌耐药率低(1%~3%)，氨基糖甙类及西林类、其他头孢类等均存在不同程度的耐药。结论：糖尿病并泌尿系感染多见于老年女性，病程长、血糖高及有急慢性并发症者好发，早期经验性选用对大肠埃希菌敏感性高的抗生素可缩短病程。[关键词] 糖尿病；泌尿系感染；耐药

Abstract:Objective Inquiry into the urinary system that diabetes mellitus combine clinical characteristics and bactrias infect with to bear the medicine circumstance.Methods To our hospital endocrine,internal secretion section from October of 1995~ October of 2004 during 9 years the hospitalization accepts 320 diabetes mellituses that cure the urinary system combines the clinical data infects with the sufferer proceeds the review analysis.Results Causing the pathogen of the infection regard the Green's surname feminine gender rod bacteria as principle,occupying 90.2%,the escherichia coli is the most familiar to occupy 65% the next in order for transform the rod bactenia,viceescherichia coli,sewerage intestine rod bacteria,etc.of aerobacter aerogenes.Factor that affect infection as:Female are obvious higher than male(P<0.01)；The old age is obvious higher than youth and prime of life(P<0.01);Disease distance elder higher than the disease distance is short;The erupts the ketosis acidosis,hyperosmolar higher than general disease;Hyperglycemia higher than hypoglycemia(P<0.01);Contain the pathological changes of bladder higher than have no the pathological changes of bladder;The have no symptom mold ketonuria higher than have the symptom.The drug sensitive indol test result shows,imipenem,cefoperazone,the sand star bactria bears the medicine rate low(1%~3%),amino sugar glycoside and west wood,other cephalosporins.all exsits the different degree bears the medicine.Conclusion The diabetes mellitus combines urinary system infection is much seen in old age female,the disease distance longus,blood sugar is high and have the nasty and chronic complications good hair,the earlier period experience choose to use to can shorten the disease distance to the high and antivirus vegetable in sensitivity in escherichia coli. ......
