【摘要】 目的：探讨腹腔镜子宫切除术不同术式的适应证及效果。方法：回顾分析220例腹腔镜子宫切除术临床资料，并与同期420例开腹子宫切除术进行对比。结果：腹腔镜子宫次全切除术(LSH)并发症发生率少于其它术式(P<0.01)，LSH与腹腔镜鞘膜内子宫切除术(LISH)适于中青年患者；腹腔镜辅助阴式子宫切除术(LAVH)及腹腔镜全子宫切除术(LTH)适用于中老年患者，但LTH手术难度大。与同期开腹子宫切除术相比，腹腔镜子宫切除术出血量及手术时间未增加，术后病率低(P<0.01)。结论：腹腔镜子宫切除术微创特点突出，不同术式各有其适应范围，应用时注意选择患者和术式。【关键词】 腹腔镜；子宫切除术；适应证；开腹手术

【Abstract】 Objective:To evaluate the clinical efficacy and indication of four different operative patterns of laparoscopic hysterectomy.Methods:A retrospective analysis on 220 cases of laparoscopic hysterectomy was carried out,including indication,operative and postoperative complications,and the clinical efficacy,The index was compared with those of 420 cases of hysterectomy.Results:Laparoscopic subtotal hysterectomy(LSH) was superior to the others(P<0.01).LSH and laparoscopic in trafascial supracerrical hysterectomy(LISH) were applied to the younger.LISH had better sexual function.laparoscopic assisted vaginal hysterectomy(LAVH) and laparoscopic total hysterectomy(LTH) were applied to the senior,but the procedure was more difficult in LTH than in LAVH.Compared with laparohysterectomy,laparoscopic hysterectomy had some advantages,and better postoperative recovery.Conclusions:Laparoscopic hysterectomy has the characteristics of minimally invasive surgery.The selection of the ideal pattern for certian patient is important. ......
