【摘要】 目的 观察小剂量美托洛尔对慢性肺心病心律失常的疗效，及其对肺通气功能的影响，评估慢性肺心病(COPD)患者对高选择性β-受体阻滞剂的用药有效性与安全性。方法 缓解期或急性加重期待病情得到有效控制后的慢性肺心病，存在心律失常的患者，给口服美托洛尔6.25～12.5mg，每12h 1次。于用药前和用药后14天分别做24h动态心电图，观察心律失常变化；并于用药前及用药后14天和28天以后分别测定肺通气功能(FVC、FEV1、FEV1%、FEF 25%～75%、FEF75%)，观察对肺通气功能的影响。结果 小剂量美托洛尔对慢性肺心病心律失常治疗有效率为100%，而用药前后肺通气功能各观察指标差异均无显著性(P>0.05)。结论 小剂量美托洛尔对慢性肺心病心律失常疗效确切，而对患者肺通气功能无明显影响，近期和长期用药均较为安全。

    【Abstract】 Objective To observe the curative effect that use small dose of metoprolol to treat the cardiac arrhythmia with chronic pulmonary heart disease, and observe the effect of lung ventilation function, evaluate the curative effect and security that use the high -select-β-receptor blocker to the patients.Methods Selection the patients with cardiac arrhythmia of chronic pulmonary heart disease in the acute or steady period, take the metoprolol 6.25～12.5mg,one dose every 12 hours, and check the ambulatory electrocardiography for 24 hours respectively before and after take metoprolol for 14 days, to observe the cardiac arrhythmia, and check the lung ventilation function(FVC、FEV1、FEV1%、FEF 25%～75%、FEF75%)respectively before take metoprolol and after take metoprolol for 14 days and 28 days, observe the effect to the lung ventilation function.Results The curative effect of small dose metoprolol to the cardiac arrhythmia with chronic pulmonary heart disease is 100%, and the differences were no significant between the two groups of lung ventilation function before and after take medicine (P>0.05). Conclusions The curative effect is affirm that use small dose metoprolol to treat the cardiac arrhythmia with chronic pulmonary heart disease but to the lung ventilation function is not effect.
