[摘要] 目的 观察左旋布比卡因与布比卡因在子宫全切术中的临床应用及效果。方法 60例病人均采用腰麻联合硬膜外穿刺，穿刺成功后蛛网膜下腔A组注入0.75%左旋布比卡因2 ml+5%葡萄糖1 ml；B组0.75%布比卡因+5%葡萄糖1 ml。结果 (1)两组病人年龄、体重、ASA分级和手术时间差异无显著性(P>0.05)。(2)两组感觉和运动阻滞起效时间及切皮、术中腹腔牵拉比较差异无显著(P>0.05)。(3)两组硬膜外用药及不良反应比较差异无显著性(P>0.25)。结论 腰麻联合硬膜外麻醉损伤小，效果可靠，血流动力学稳定，术中容易管理，并发症少，麻醉维持时间长，是临床妇科手术的一种较好的麻醉方法。

    [Abstract] Objective To observe the clinical application and effect of levobupivacaine and bupivacaine in the hysterectomies.Methods Arachnoid anaesthesia and extradural puncture were performed on 60 patients.Group A: 0.75% levobupivacaine 2 ml plus 5% glucose 1 ml.Group B:0.75% bupivacaine plus 5% glucose 1 ml.Results (1)There are no distinct difference between the two groups of patients in the age,weight,ASA levels and the surgical time(P>0.05).(2)There are no distinct difference between the two groups of patients in the beginning of effect on the sense and athletic nerves,skin incising and celiac pulling(P>0.05).(3)There are no distinct difference between the two groups of patients in medicine amount and the side effects(P>0.05).Conclusion Arachnoid anaesthesia and extradural puncture with less harm,reliable effect,stable hemo dynamics,less side effects,easy management in the surgery,longer anaesthesia time,is a desirable anaethesia method in the clinical gynaecology surgery. ......
