静脉注射右旋糖酐氢氧化铁对血透患者肾性贫血的疗效观察

[摘要] 目的 比较静脉注射右旋糖酐氢氧化铁和口服琥珀酸亚铁治疗血液透析患者肾性贫血的疗效和不良反应。方法 选择60例HD患者，分为静脉补铁组和口服补铁组，每组30例。静脉补铁组：每次透析结束前1 h注射100 mg右旋糖酐氢氧化铁，从透析器静脉端输入，时间维持1 h；口服补铁组：琥珀酸亚铁(速立菲)200 mg，每日3次，连续口服2个月。两组同时使用基因重组人红细胞生成素(rHuEPO)100~150 u/(kg·W)皮下注射。比较两组患者贫血治疗的效果和副作用。结果 治疗后两组患者贫血均改善，其中静脉补铁组的Hb、Hct及TAST较口服补铁组显著升高(P<0.01)，而不良反应发生率较口服补铁组降低。结论 静脉注射右旋糖酐氢氧化铁可作为血透伴缺铁患者更理想补铁方式，配合rHuEPO治疗贫血，疗效明显优于口服补铁，且不良反应发生率低。[关键词] 右旋糖酐氢氧化铁；肾性贫血；血液透析

[Abstract] Objective To compare with curative effect and adverse reaction on renal anemia of hemodialyses patients with Fe(OH)3dextran(CosmoFer) venously and ferromyn orally.Methods 60 HD patients were divided into venous group and oral group randomly and there were 30 patients in each group，100 mg CosmoFer were used one hour by vein point of dialyser before HD was completed in hemodialyses patients in venous group 200 mg amber acid subiron was taken by the patients in oral group three times a day and two months in total.Recombinant human erythropoietine (EPO) was used at a dose of 100～150 u/kg·W by hypodermic in each group ......
