冠心病左室心肌纵向收缩位移和同步化运动的定量研究

[摘 要] 目的 应用组织追踪(TT)和组织同步成像(TSI)技术评价冠心病(CAD)左室心肌纵向收缩位移和同步化运动的特点。方法 应用TT和TSI获取20例CAD患者和26例正常人的左室心尖长轴、心尖二腔和心尖四腔切面,测定并比较左室长轴方向不同室壁节段的收缩期最大位移(Ds)和收缩期正向达峰时间(Ts)。结果 CAD患者TT和TSI检出的心肌异常节段与冠脉造影证实或心肌梗死的病变部位基本一致;CAD患者的Ds较正常组明显降低(P<005或P<001),Ts测值延长,各节段之间的Ts均值差异增大(7148~18200ms,P<005或P<0001);以基底段Ds<9mm,中间段<5mm和Ts>180ms为异常室壁节段的标准,其准确性分别为6200%和5417%,敏感性为5263%和3944%,特异性为7442%和7551%。结论 TT和TSI能较好地评价CAD患者左室心肌纵向收缩时的位移变化和同步化。[关键词] 组织追踪;组织同步成像;冠心病;左心室

[Abstract] Objective To evaluate the longitudinal myocardial displacement(Ds) and synchronization motion features of LV ontraction in coronary artery disease(CAD) using the tissue tracking (TT) and tissue synchronization imaging(TSI)Methods The LV apical long axis view,apical twochamber view and fourchamber view were obtained by TT and TSI in 20 CAD and 26 normal subjectsPeak systolic Ds and systolic time to peak(Ts)were measured and compared between the two groupsResults The abnormal segments detected by TT and TSI were consistent with coronary angiography and myocardial infarction in CAD groupDs was signiicantly lower in CAD group than that of the corresponding segments in normal subjects (P<005 or P<0001)Ts was delayed and the difference of Ts among segmentswas significantly increased in CAD patients (7148~182ms,P<005 or P<0001)Conclusion TT and TSI can more accurately evaluate the LV myocardial longitudinal contraction displacement and synchronization in CAD patients ......
