【摘 要】 目的 分析比较脑卒中(脑出血)伴热结旁流的临床特征和危险因素。方法 比较20例脑出血伴热结旁流患者与40例急性肠炎患者临床表现，与40例脑出血不伴热结旁流患者1∶2配对条件Logistic回归分析危险因素。结果 前驱便秘，第一主诉腹泻，大便量少、次数多、无色、稀薄黏液、粪质均匀无渣、混有燥结粪粒，触及腹块，热结旁流组多于急性肠炎组(P<0.05)。腹痛、肠鸣音亢进、低血压、大便色黄或青，热结旁流组少于急性肠炎组(P<0.05)。镜检白细胞、发热、腹胀、矢气多，2组差异无显著意义(P>0.05)。对7例热结旁流患者进行了肛门指检，均可触及硬结粪块。条件Logistic回归分析结果表明，禁食且无鼻饲、阴虚体质、20%甘露醇大量应用和口服抗生素是引发脑出血伴热结旁流的独立危险因素。结论 脑出血伴热结旁流具有特征性临床表现和明确的危险因素。【关键词】 热结旁流；脑出血；临床特征；危险因素
Clinical features and risk factors in stroke patients complicated by retention of heatevil with watery diarrhea

【Abstract】 Objective To investigate the clinical features and risk factors of retention of heatevil with watery diarrhea (RHWD) associated with stroke(intracerebral hemorrhage).Method The comparison of clinical data was made between RHWD(20 patiens) and acute intestinitis(40 patiens). The risk factors of RHWD were analyzed by 1∶2 matched pair via conditional Logistic regression in intracerebral hemorrhage patients with and without RHWD.Results The antecedent constipation, diarrhea for first chief complaint, stool chracteristics of decreased volume, increased frequency ,thin mucus, colourless, dregless, dry feces granules and abdominal mass were frequently in the RHWD group (P<0.05), but abdominal pain, rumbing noise and low blood pressuse were less common (P<0.05).The fecal white blood cell count,fever, abdominal flatulence and break wind showed no difference in two groups (P>0.05).7 patiens were detected by rectal examination and the dry feces mass could be found in all cases. The fasting without gastric tube feeding, yindeficiency, mannitol in big dosage and via oral antibiotics were revealed by Logistic regression analysis to be the independent risk factors related with RHWD.Conclusion There are characteristic clinical features and definite risk factors in RHWD associated with intracerebral hemorrhage. ......
