老年高血压患者心脏结构和功能与年龄相关性分析

     【摘要】目的通过对老年高血压(EH)患者合并2型糖尿病(T2DM)、冠心病(CAD)心脏结构与功能的改变及其与年龄的相关性分析，探讨老年EH影响心脏结构与功能的主要因素。方法老年EH患者169例，年龄60~97岁,平均(758±998)岁，按EH有无合并T2DM、CAD分为4组，单纯EH组40例，EH+CAD组66例，EH+T2DM组22例，EH+T2DM+CAD组41例。常规探查心脏结构指标：左心房直径(LAD)、左心室后壁(LVPW)厚度。左室收缩功能指标: 左室射血分数(EF)、左室最大长轴缩短率(FS)。左室舒张功能指标: EF 斜率。结果 4组LAD、LVPW厚度、FS 和EF均无显著性差别，单纯EH组和EH+T2DM+CAD组EF斜率有显著性差别(P<005)。LAD、LVPW厚度与年龄呈正相关，FS、EF和EF斜率与年龄呈负相关。结论 EH合并T2DM、CAD后，与单纯EH相比，虽然心脏结构无明显变化，但EF斜率明显降低，舒张功能进一步受损。随着年龄的增加，LAD和LVPW厚度显著增加，而FS和EF斜率与年龄呈负相关，并且收缩功能在60~69岁组和≥90岁组比较有显著性差别，舒张功能从60岁开始每增加10岁都有显著降低。

    【Abstract】 Objective To investigate the relationship of cardiac structure and function with age in elderly patients with essential hypertension(EH) and diabetes mellitus(DM) or coronary artery disease(CAD). Methods Twodimensional echocardiography studies were performed on 169 patients with EH (mean age 7577±998 years), including 40 cases of only EH, 66 cases of CAD and EH, 22 cases of DM and EH, and 41 cases of DM and CAD and EH. Resukts In the four groups, the parameters including the left atria dimension(LAD), the diastolic thickness of the left ventricular posterior wall(LVPW) were significantly increased with the increase of age and the percentage of cardiac systolic or diastolic dysfunction was significantly decreased with the increase of age. Conckusions The changes of cardiac structure and function of elderly EH patients with T2DM or (and) CAD were significantly correlated with age. ......
