[摘要] 目的 探讨无张力疝修补术治疗腹外疝的临床效果。方法 回顾分析2003年7月~2005年12月无张力疝修补78例(A组)，传统疝修补417例(B组)，对比分析两组临床资料。结果 (1)平均手术时间：A组50 min，B组61 min；两组差异无显著性(χ2=3.0356，P>0.05)；(2)平均住院日：A组4.3天，B组9.1天，两组差异有显著性(χ2=10.5996，P<0.005)；(3)平均医疗费用：A组3980.2元，除外材料费2000.2元，B组2097.13元，两组差异无显著性(χ2=2.056，P>0.05)；(4)平均术后下床活动时间：A组12.4 h，B组192.4 h，两组差异有显著性(χ2=11.3459，P<0.005)；(5)平均术后恢复日常活动时间：A组4天，B组59天，两组差异有显著性(χ2=13.1575，P<0.005)；(6)术后并发症：A组29.5%，B组57.3%，两组差异有显著性(χ2=19.3209，P<0.005)；(7)术后复发：A组1.17%，B组18.0%，两组差异有显著性(χ2=5.002，P<0.05)。结论 腹外疝无张力疝修补平均手术时间、除外修补材料的平均医疗费用与传统疝修补相等；平均住院日、平均术后下床活动时间、平均术后恢复日常活动时间、术后并发症及术后复发率明显低于传统疝修补术；降低修补材料费后无张力疝修补术是治疗中老年腹外疝、复发疝及有并发症的腹外疝的最佳术式。
