甲状腺癌组织中galectin3、CK19的表达及应用价值

摘要:目的: 探讨半乳糖凝集素3(galectin3)和细胞角蛋白19(CK19)在甲状腺良、恶性病变组织中的表达及应用价值。方法: 采用免疫组织化学方法(SP法)检测68例甲状腺良、恶性病变组织中galectin3 、CK19的表达情况,比较有、无淋巴结转移甲状腺癌组织中galectin3、CK19的表达差异,并计算检测的灵敏度和特异度。结果:30例甲状腺癌组织中galectin3、CK19阳性表达率分别为90.0%、96.7%,38例甲状腺良性病变组织中阳性表达率分别为15.8%、7.9%,良、恶性病变组织阳性表达率差异有统计学意义(P<0.05)。淋巴结转移甲状腺癌组织中galectin3的阳性表达率为100.0%,无淋巴结转移甲状腺癌组织的阳性表达率为62.5%,差异有统计学意义(P<0.05)。淋巴结转移甲状腺癌组织CK19的阳性表达率为100.0%,无淋巴结转移甲状腺癌组织的阳性表达率为87.5%,差异无统计学意义(P>0.05)。galectin3联合CK19检测甲状腺良、恶性病变的灵敏度、特异度分别为90.0%、92.1% ,单独检测galectin3的灵敏度、特异度分别为90.0%、84.2%、96.7%、92.1%。结论: galectin3的表达与颈淋巴结转移有关,对预测甲状腺癌的发展及淋巴结转移有参考价值。galectin3和CK19在甲状腺良、恶性病变组织中的表达基本平行,二者联合检测有助于鉴别甲状腺良、恶性病变。关键词: 甲状腺; 良、恶性病变; 细胞角蛋白19; 半乳糖凝集素3; 免疫组织化学
Studing the value of galectin3, CK19 expression in the thyroid cancer

Abstract: Objective: To measure expression of galectin3, CK19 in specimens taken from the benign or malignant disease of thyroid gland. To study the value of combination detection with galectin3, CK19 in distinguishing the benign or malignant disease of thyroid gland. Methods: Immunohistochemistry (SP method) was employed in this study to detect the expression of galectin3, CK19 in the benign or malignant disease of thyroid gland. Results: (1) Expression of galectin3 was positive in 27 of 30 cases of thyroid carcinoma, the rate was 90.0%, the positive rate of benign disease was 15.8%, Compare between the benign or malignant disease of thyroid gland pathologically, the difference was high significance (P<0.05). CK19 expressed positive in 29 cases of thyroid adenocarcinoma, the positive rate was 96.7%, the positive rate in the benign disease of thyroid gland was 7.9%, the difference had high significance (P<0.05) in two groups compared. The expression of galectin3 had significance (P<0.05) with metastasis of lymph node of neck. The sensitivity, specificity were 90.0%, 92.1% respectively in distinguishing between the benign and malignant disease of thyroid gland employed in combination detection with galectin3 and CK19. While measuring galectin3 alone, the sensitivity, specificity were 90.0%, 84.2% respectively. Conclusions: The expression of galectin3 has close correlation with metastasis of lymph node of neck. Combination detection with galectin3 and CK19 can distinguish the benign or malignant disease of thyroid gland effectively. ......
