摘要:目的 评价 18 F-FDG双探头符合线路显像及其同机CT图像融合在鼻咽癌患者中临床应用价值。 方法 89例鼻咽癌(NPC)病人(初发38例、放疗或放化疗后病人51例)进行 18 F-FDG SPECT显像及同机CT图像融合检查。结果与同期CT/MRI显像结果和3～6个月的临床追踪结果比较。 结果 18 F-FDG SPECT符合显像诊断鼻咽部恶性病灶的敏感性、特异性和准确率分别为94.4%、85.7%和91.0%。解剖显像的诊断效能分别为77.8%、77.1%和77.5%(P<0.05)。其中FDG SPECT符合显像鉴别鼻咽癌放疗后的复发或残存敏感性、特异性和准确率分别为87.5%、85.7%和86.3%，解剖显像则分别为62.5%、77.1%和72.6%(P<0.05); 18 F-FDG显像有效地进行NPC临床分期，在初发NPC患者中6例分期上调，占(6/37)16.7%，7例分期下调，占(7/37)25%。对治疗后的患者能有效地评价治疗效果;同机CT图像融合能提供较可靠的定位诊断。 结论 18 F-FDG SPECT显像在NPC诊断及分期中有实用价值，值得临床广泛应用。
