比较Twin-Block矫治器与直丝弓矫治器配合口外弓及Ⅱ类牵引早期矫治骨性安氏Ⅱ类1分类错牙合临床效果 (pdf)

    【摘要】 目的 对比Twin-Block矫治器与直丝弓矫治器配合口外弓及Ⅱ类牵引早期矫治骨性安氏Ⅱ类1分类错牙合的临床效果。方法 选择处于生长发育高峰期的患者30例。纳入标准：面型为凸面型,ANB≥5°，磨牙、尖牙远中关系。分为两组，每组15例。分别用Twin-Block矫治器及直丝弓矫治器配合口外弓及Ⅱ类牵引治疗。对比两组患儿治疗前后头颅侧位片，并作统计学分析。结果 矫治结束后，所有患儿磨牙和尖牙均达到或接近中性关系，ANB角减少至正常范围。Twin-Block矫治器的骨效应明显大于直丝弓矫治器，使用Twin-Block矫治器，患儿下颌骨体长度及下面高增加更显著。 结论 Twin-Block矫治器对尚处于生长发育期的早期骨性Ⅱ类错牙合患者疗效显著，对于下颌后缩显著，上切牙较为唇倾，下切牙较为直立的病例侧貌改善效果更佳。

    【Abstract】 Objective To investigate the early treatment effects of Twin-Block appliance and strightwire appliance combined of cervical headgear and Class Ⅱ division 1 malocclusion cases.Methods The study consisted of pre-and post-treatment from 30 cases,including convex profile;ANB ≥5°;molar and canine Class Ⅱ relationship.The cases were divided into 2 group randomly:15 of them were treated with Twin-Block while the others with strightwire appliance.The cephalometry of pre-and post-treatment was measured and analyzed using the computer software.Results Molar and canine relationship changed to Class Ⅰ for all of the cases after the treatment and ANB became normal.The effects of the Twin-Block appliance in the correction of Class Ⅱ malocclusion were primarily on skeletal changes.The mandibular length and the lower anterior facial height were significantly increased in the Twin-Block group compared to the strightwire appliance group.Conclusion Twin-Block appliance can be used successfully for early treatment of skeletal Class Ⅱ malocclusion.It is more indicated for patients with retracted mandible,more labially inclined upper incisors and more upright lower incisors. ......
