18FFDG PET与符合线路SPECT NSCLC纵隔淋巴结转移对比研究 (pdf)[摘要] 目的 比较18FFDG PET和18FFDG符合线路SPECT在NSCLC纵隔淋巴结转移及其分期中的临床应用价值。方法 临床诊断为肺部恶性肿瘤,怀疑有纵隔及肺门淋巴结转移患者20例,同天依次接受PET和符合线路SPECT检查。分别由两位有经验的核医学医师进行诊断。检查后1~2周接受手术或纵隔镜检查获得病理诊断。结果 20例患者中,18例恶性病变,出现肺门及纵隔淋巴结转移12例。获得病理诊断淋巴结220枚,转移淋巴结51枚,无转移169枚。PET和SPECT的灵敏度、特异度、准确度分别为94.12%(48/51)、90.53%(153/169)、91.36%(201/220)和82.35%(42/51)、92.90%(157/169)、90.45%(199/220)。PET灵敏度高于符合线路SPECT且差异有显著性(P<0.05),其他指标差异无显著性(P>0.05)。其中PET真阳性而SPECT假阴性的淋巴结6枚,直径分别为11.0 mm,8.0 mm,12.0 mm,10.5 mm,13.8 mm和14.8 mm。结论 18FFDG PET和符合线路SPECT均可用于非小细胞肺癌纵隔淋巴结转移探测及分期,当转移淋巴结≥15 mm时,两者有较好的一致性;在显示纵隔结构和早期发现微小转移淋巴结方面,PET优于符合线路SPECT。

Comparison of 18FFDG PET and coincidence SPECT in mediastinal lymph nodes staging of NSCLC

[Abstract] Objective To compare 18Ffluorodeoxyglucose(18FFDG)positron emission tomography(PET)with 18FFDG coincidence single photon emission computed tomography(SPECT)in the mediastinal lymph nodes staging of nonesmall cell lung cancer(NSCLC).Methods 20 patients with spaceoccupying lesions in lung were examined with 18FFDG PET imaging and had the SPECT imaging after the PET scanning 1~2 hours.Two practiced nuclear medicine doctor made the diagnoses respectively.The histopathology diagnoses were obtained either by thoracotomy or mediastinoscopy in 1~2 weeks.Results 18 of the 20 patients with malignant lung tumors by surgical procedures were metastasis in 12 patients,and nonmetastasis in 6 patients.Among the 220 lymph nodes with histological results,51 were involved and 169 were not.The sensitivity,specificity,accuracy were obtained respectively:PET,94.12%(48/51), 90.53%(153/169) and 91.36%(201/220);SPECT,82.35%(42/51),92.90%(157/169)and 90.45%(199/220).The sensitivity of PET was significantly higher than SPECT(P<0.05).The diameter of the lymph nodes which were true positive in PET and false negative in SPECT are 11.0 mm,8.0 mm,12.0 mm,10.5 mm,13.8 mm and 14.8 mm.Conclusion SPECT reveals similar results in comparison with PET when metastatic lymph nodes ≥15 mm.PET has superiority in showing the mediastinal structure and revealing minute metastatic lesions in earlier period. ......
