    Abstract

    Ulcerative colitis, which is fairly difficult to be treated, has an extended course and frequent recurrent rate. As a new direction in the pathogenesis of ulcerative colitis, the role of intestinal motility is being recognized and becoming a hotspot. It is reported that the changes of number and structure in interstitial cells of Cajal and colonic manometry, the abnormity of electrophsiology in the intestine and the expression of neurotransmitters, including acetylcholine (Ach), vasoactive intestinal peptide (VIP), nitric oxide (NO), and carbon monoxide (CO), are associated with the pathogenesis of ulcerative colitis. Further investigation of intestinal motility has an important significance in realizing the pathogenic mechanism of ulcerative colitis.

    摘要

    溃疡性结肠炎(ulcerative colitis, UC)治疗难度大, 疗程长, 而且治愈后常易复发. 近年来, 肠道动力学异常与UC的发病关系得到医学界的重视, 并逐渐成为UC发病机制研究的热点. Cajal间质细胞结构改变与数量异常、肠道电生理紊乱、结肠压变化, 以及肠道相关神经递质(Ach、VIP、NO、CO等)表达异常与UC的发病有关. 相关肠道动力学异常参与UC发病的具体机制则有待于进一步深入研究.

    0 引言

    溃疡性结肠炎(ulcerative colitis, UC)是以腹痛、腹泻、黏液脓血便、里急后重等为主要症状, 以直、结肠黏膜及黏膜下层炎症和溃疡形成为病理特点的一种慢性非特异性消化道疾病. 其病因可能与遗传, 肠道菌群, 免疫失调, 饮食过敏, 精神抑郁及焦虑等有关. 本病治疗难度大, 疗程长, 而且治愈后常易复发. 尤其是UC病程较长, 病变范围广泛, 病情反复发作者, 被认为结肠癌的癌前病变. 本病的发病率和患病率在我国有明显增加趋势[1]. 近年来, 肠道动力学异常与UC的发病关系得到医学界的重视, 并逐渐成为UC发病机制研究的热点. 本文就目前溃疡性结肠炎的肠道动力学研究进展作一综述.

    1 Cajal间质细胞与溃疡性结肠炎

    1.1 ICC的概念 Cajal间质细胞(interstitial cells of Cajal, ICC)是胃肠道内一种与神经系统密切相关的特殊类型的间质细胞, 其主要功能有[2-3]: (1)胃肠平滑肌的起搏细胞, 能产生胃肠道慢波(slow wave, SW), 控制胃肠道平滑肌的收缩和蠕动; (2)胃肠道非肾上腺素能, 非胆碱能抑制性神经传递的介质; (3)促进电活动扩布. 研究证实, Cajal间质细胞在胃肠道电生理和动力发生以及动力障碍上具有重要作用[4] ......
