【摘要】 目的 探讨无创机械通气(NIV)在慢性阻塞性肺疾病急性加剧致急性呼吸衰竭患者中的应用价值。方法 20例重度高碳酸血症呼吸衰竭(HRF)患者进行双水平气道正压通气(BiPAP),监测治疗前后动脉血气(pH、PaCO2、PaO2)。结果 20例重度高碳酸血症呼吸衰竭(HRF)患者中，15例重症呼吸监护病房(RICU)面罩BiPAP成功患者治疗前动脉血气(pH 、PaCO2、PaO2)和RR(呼吸频率)分别为：7.23±0.05、104±14mmHg、84±30mmHg和34.6±8.5次/min，治疗后分别为：7.38±0.05、67±8mmHg、73±5mmHg和18±3次/min，两者差异有显著性(P<0.05)。结论 无创机械通气治疗慢性阻塞性肺疾病急性呼吸衰竭有显著疗效。

    The application value of noninvasive mechnical ventilation in patients of chronic obstructive pulmonary disease complicated with acute respiratory failure

    【Abstract】 Objective To discuss the value of noninvasive mechanical ventilation (NIV) in the practice of the patients who have acute respiratory failure which was accumulated urgently from chronic obstructive pulmonary disease(COPD).Methods 20 patients with serious hypercapnic respiratory failure (HRF) have been treated with bi-level positive airway pressure(BiPAP), the arterial blood gas before and after the treatment have been detected for analysis.Results Among the 20 patients, 15 ones which are in RICU with BiPAP have been improved effectively.
