应用小球囊收取脱落支架(摘要)

方法和结果:截止1999年7月20日,我院共为1 032例患者置入1 472枚支架。选择置入支架的连续病例中,3例患者(0.3%)支架脱落在冠状动脉内。2例患者为右冠状动脉PTCA,1例为回旋支PTCA。2例为B2型病变,1例为C型病变。置入支架的指征:2例为夹层,1例为扩张结果不满意。2个支架脱落在右冠状动脉近段,1个支架脱落在左主干和回旋支之间。3例患者均使用相同的方法收取脱落支架,即用1.5 mm的小球囊穿过支架,加压至3~4个大气压,将脱落的支架和小球囊一起撤出,均告成功,且无并发症。
结论:小球囊收取脱落支架是一种安全、有效的方法。
Using a Small Balloon for Intracoronary Lost Balloon-Monted Stent (Abstract)

Objective: Loss of Intracoronary balloon-mounted stents prior to deployment is a rare event but cardiac ischemia or peripheral embolization may occur during rescue attempts. This study was to assess the efficacy of small balloon retrieval for the lost stents.
Methods and Results: Until July 20, 1999, stent was deployed in 1 032 patient with 1 472 stents. In a consecutive series of our patients undergoing elective stenting, three instances of intracoronary stent loss (0.3%) were encountered. PTCA was performed for right coronary artery in two patients and for circumflex in one. There were B2 lesion for two patients and C lesion for the third. Indications for stenting were dissection for two patients and suboptimal result for one. Two stents were lost in proximal right coronary artery and one was dislodged between left main coronary artery. In all these patients, we used the same method to get the lost stents. Successful percutaneous retrieval for the lost stents was achieved using a 1.5 mm balloon. The small balloon crossed the stent and then was inflated with 3~4 atms. The small balloon was successfully withdrawn with the lost stent without any complications.
Conclusion: Small balloon for the dislodged stent is a safe and effective method., 百拇医药
