    方法:应用小C壁X光机以标准方法对87例冠心病患者进行了PTCA和冠状动脉内支架置入术。并对所有患者进行了(1~18)个月的随访。

    结果:87例患者共扩张99支血管,112处病变,病例成功率95.4%,血管成功率94.0%,病变成功率93.1%。完全血运重建53例,部分血运重建34例。置入支架62枚,其中前降支36枚,回旋支9枚,右冠17枚。择期PTCA 65例,冠状动脉造影和PTCA术同时进行18例,挽救PTCA 2例,直接PTCA 2例。术中死亡1例,2枚支架脱落于冠状动脉内,均被摘出体外。边支闭塞2例,术后1天再梗塞1例,出现腹膜后巨大血肿行外科手术1例。随访时,心绞痛复发6例,原部位再发心肌梗塞1例,无死亡发生。

    结论:小C臂X光机用于PTCA和支架置入术治疗是可行和相对安全的。

    Percutaneous Transluminal Coronary Angioplasty by Using the Portable Fluoroscopic Imaging System (A Report of 87 Cases) (Abstract)

    Objective: To evaluate the feasibility and safety of percutaneous transluminal coronary angioplasty (PTCA) and intraccoronary stenting by using the portable fluoroscopic imaging system.

    Methods: From January 1, 1988 to August 20, 1999, 87 patients with a presence of ≥75% diameter luminal narrowing in one or more of major epicardial vessels underwent PTCA and intracoronary stenting. Clinical follow up were performed 1~18 months after PTCA in all patients.

    Results: PTCA was performed in 87 patients with 99 diseased vessels and 112 lesions, 36 stents were implanted into LAD, 9 stents into LCX and 17 stents into RCA. The clinical (patient), technical (vessel), and lesion success rate was achieved in 95.4%, 94.0%, and 93.1%, respectively. Fifty-three patietns had complete vessel revascularization of the ischemicrelated vessels and 34 patients had imcomplete vessel revascularization. Staged PTCA was performed in 65 patients, one-stage of coronary angiography and PTCA in 18 patients, salvage PTCA in 2 patients and direct PTCA in 2 patients. During PTCA, death occurred in one patients, 2 stents exited from the balloon before delivered to the target lesion were retrieved with microsnare retrieval system, side branch occlusion occurred in 2 patients, reinfarction in one after PTCA, and large post-peritoneal hematoma was observed and operation was performed in one. During a mean 12-month clinical follow-up, all patients survived. The angina recurrence rate was 6.90% (6 of 87 patients), one patients had reinfarction in primary lesion.

    Conclusion: PTCA and intracoronary stenting by using portable fluoroscopic imaging system is effective and relative safe for the management of coronary artery disease., 百拇医药
