用含大隐静脉的带蒂皮瓣修复胫前创面

潍坊医学院000117摘 要：目的探讨含大隐静脉的带蒂皮瓣修复胫前创面的手术治疗效果。方法于病人小腿前内侧设计含大隐静脉及隐动脉的L形皮瓣，为12例病人修复胫前创面，创面最大5cm×3.5cm，最小2cm×1.5cm。结果术后皮瓣全部成活，切口Ⅰ期愈合；随访5个月到2年，皮瓣生长良好，创面未复发。结论本皮瓣含大隐静脉、隐动脉及较宽的皮蒂三套供血系统，较岛状皮瓣及任意带蒂皮瓣血供更好，既有利于皮瓣成活，又有利于控制感染，是修复胫前直径在5.0cm以下有较多瘢痕组织创面的理想皮瓣。

Pedicle Flap with saphenous Vein for Wound in Front of Tibia

Abstract：Objective To investigate the operative method and clinical result of the pedicle flap with saphenous vein for wound in front of tibia.Method According to the anatomic study of adult cadavers,the L-shape and tongue-shape flap with saphenous vein were designed.12 cases who had a wound in front of tibia which was 2cm×1.5cm to 5cm×3.5cm were treated with the operative ways.Result All the flaps survived,and the wound heald with I stage.Follow-up was from 5 months to 2 years.The flaps were growing well and the wound had no recurrence.Conclusion The flap contain three kinds of blood supply system from saphenous vein,saphenous artery and a wider skin pedicle.So its blood supply is very reliable and suitable to repair the wound in front of tibia which is smaller than5.0cm in diameter.
