产前超声诊断胎儿右位主动脉弓34例临床分析(3)
综上所述,产前胎儿心脏系统筛查中,三血管气管切面是检查者发现右位主动脉弓的第一提示平面。当怀疑右位主动脉弓诊断时,要仔细扫查主动脉弓全程及分支,并谨慎与双主动脉弓和镜像颈动脉分支相鉴别。需要警惕是否合并室间隔缺损或圆锥动脉干畸形等心内畸形,以及是否合并22q11缺失等染色体核型异常。对于血管环畸形的预后,并不是形成血管环就会有压迫症状,临床预后主要决定于合并畸形和染色体异常情况。孤立性右位主动脉弓合并左位动脉导管形成血管环, 但大部分预后较好,建议继续妊娠。此外,一旦诊断右位主动脉弓,应进行详尽的心内心外超声扫查及染色体核型分析[6-7]。
