呼吸湿化治疗仪对慢性心力衰竭伴低氧血症患者血气分析指标及D-二聚体水平的影响(1)
【摘要】 目的 观察呼吸湿化治疗仪对慢性心力衰竭伴低氧血症患者血气分析指标、D-二聚体水平的影响。方法 选择68例慢性心力衰竭伴低氧血症患者,按照入院顺序分为观察组与对照组,每组各34例患者。2组患者均予相同的常规基础治疗,在此基础上对照组予鼻导管高流量吸氧,观察组予呼吸湿化治疗仪治疗。观察2组治疗前后血气分析指标PaO2、SaO2、氧合指数(PaO2/FiO2)、动脉血碳酸氢根(HCO3-)、PaCO2、动脉血酸碱度值(pH)的变化及组间差异,并分析治疗前后2组患者外周血D-二聚体水平。结果 治療后,2组PaO2、SaO2、PaO2/FiO2、pH值均比治疗前升高,HCO3-、PaCO2、D-二聚体水平均比治疗前下降(P均< 0.05)。治疗后观察组PaO2、SaO2、PaO2/FiO2、pH值均高于对照组,HCO3-、PaCO2、D-二聚体水平均低于对照组(P均< 0.05)。结论 呼吸湿化治疗仪能有效改善慢性心力衰竭伴低氧血症患者的缺氧状态,降低D-二聚体水平,有助于缓解肺水肿。

【Abstract】 Objective To observe the effect of respiratory humidification treatment device on blood gas analysis parameters and D-dimer levels in patients with chronic heart failure complicated with hypoxemia. Methods Sixty-eight patients with chronic heart failure complicated with hypoxemia were enrolled and divided into the observation (n = 34) and control groups (n = 34). On the basis of routine therapy, high-flow nasal oxygen therapy was given in the control group and respiratory humidification treatment device was utilized in the observation group. The changes of arterial blood oxygen pressure (PaO2), arterial oxygen saturation (SaO2), oxygenation index (PaO2/FiO2), arterial blood carbon bicarbonate (HCO3-), arterial blood carbon dioxide pressure (PCO2) and arterial blood pH value (pH) were statistically compared between two groups before and after corresponding treatment. The D-dimer levels in the peripheral blood were analyzed between two groups before and after therapy. Results After corresponding treatment, the values of PaO2, SaO2, PaO2/FiO2 and pH in two groups were significantly increased (all P < 0.05), whereas the levels of HCO3- and PCO2 and D-dimer were significantly decreased (all P < 0.05). After corresponding treatment, the values of PaO2, SaO2, PaO2/FiO2 and pH in the observation group were significantly higher, whereas the levels of HCO3- and PCO2 and D-dimer were remarkably lower compared with those in the control group (all P < 0.05). Conclusion Respiratory humidification treatment device can effectively mitigate hypoxemia, lower the D-dimer levels and contribute to alleviating the symptoms of pulmonary edema., 百拇医药(杜璐玲?陈建永?陈楚芬)
