葡萄膜炎患者视力受损程度对生活质量和心理状态的影响(1)
【摘要】目的探讨葡萄膜炎患者视力受损程度对生活质量和心理状态的影响。方法以2016年9月～2017年11月收治的105例葡萄膜炎患者作为研究对象，根据视力受损程度进行分组，任一眼视力≤0.3的44例为≤0.3组，双眼视力>0.3的61例为>0.3组。采用视觉相关生活质量量表(NEI-VFQ-25)及焦虑抑郁量表分别对两组患者的生活质量和心理状态进行评估，并对结果进行比较分析。结果≤0.3组患者总体视力、近距离活动、远距离活动、社会活动、精神健康、角色困难、依赖性/独立性、驾驶车辆、色觉及总均分方面的评分均较>0.3组患者明显降低(P<001)；≤0.3组患者焦虑、抑郁评分均明显高于>0.3组(P<0.05)，且≤0.3组焦虑、抑郁患者相比>0.3组患者明显增加(P<0.05)；经Pearson相关性分析显示，焦虑评分、抑郁评分均与视觉相关生活质量总均分呈负相关(P<001)。结论葡萄膜炎患者视觉相关生活质量水平普遍低下，视力损害程度越高其生活质量越差，而因疾病引发的焦虑抑郁情绪对于生活质量水平也有着较大的影响。

【Abstract】ObjectiveTo investigate the effect of visual impairment on the quality of life and mental state in patients with uveitis.Methods105 cases of uveitis patients admitted to hospital from September 2016 to November 2017 were selected as subjects and divided into groups according to the degrees of visual impairment.44 cases with visual acuity≤0.3 in any eye were selected as≤0.3 group，and 61 cases with binocular vision>0.3 were selected as>0.3 group.The quality of life and psychological status of the two groups were evaluated by NEI-VFQ-25 and anxiety depression scale，and the results were compared and analyzed.ResultsThe scores of overall visual acuity，close range activities，long distance activities，social activities，mental health，role difficulties，dependence/independence，vehicles driving，color vision and total mean score of the ≤03 group were significantly lower than those in the >0.3 group(P<0.01)；the scores of anxiety and depression in the ≤0.3 group were significantly higher than those in the >0.3 group(P<0.05)，and the patients with anxiety and depression in the ≤0.3 group significantly increased compared with those in the >0.3 group(P<0.05).Pearson correlation analysis showed that anxiety scores and depression scores were negatively correlated with total visual-related quality of life(P<0.01).ConclusionThe visual-related quality of life in patients with uveitis is generally low.The higher the degree of visual impairment，the worse the quality of life，and the anxiety and depression caused by disease also have a great impact on the quality of life.

葡萄膜炎是炎癥累及包括虹膜、睫状体、视网膜、玻璃体等多个眼部组织的常见眼科疾病，根据发病部位的不同临床分为前葡萄膜炎、中间葡萄膜炎、后葡萄膜炎及全葡萄膜炎，其中前葡萄膜炎最为常见，多见于男性[1]。由于该病对视力损伤较大，10%～15%的盲人是因葡萄膜炎致盲[2]，因而该病对于患者的生活、工作和学习均有着严重的影响。随着社会的进步和科技的发展，临床医学模式逐渐向生物-心理-社会医学模式转变，即在一定程度上，健康和疾病是生物学变化过程，但同时心理和社会因素也是影响疾病的主要因素，三者相互影响。这就要求在临床治疗的过程中不仅仅需要关注机体的健康，还应加强对心理健康和社会因素的调节[3～4]。鉴于此，本研究旨在通过对不同病情严重程度葡萄膜炎患者生活质量和心理状态的分析，探讨病情严重程度对心理和生活质量的影响，以便临床制定具有针对性的干预措施，提高临床治疗的整体效果。, 百拇医药(张行行 李玉霞)
