高血压合并冠心病患者检测尿微量白蛋白\血清胱抑素C的临床意义(1)

     摘要:目的 探讨尿微量白蛋白(Microalbuminuria, MAU)和血清中胱抑素C(Cystatin C,CysC)在高血压合并冠心病患者中的诊断意义。方法 选择130例拟诊冠心病的高血压病患者,根据冠脉造影结果分为冠心病Ⅰ组(单支病变组)、Ⅱ组(冠脉病变血管支数≥2)、及单纯高血压组,分别采用免疫投射比浊法测定各组尿微量白蛋白浓度,酶联免疫吸附法检测各组血清中胱抑素C的含量;同时以正常人群组作为对照,比较分析不同组别MAU、血清中CysC含量的差异及其所代表的临床意义。结果各冠心病组的MAU、血清中CysC含量、单纯高血压组较正常人组显著升高(P < 0.05),冠心病Ⅱ组和冠心病Ⅰ组均高于高血压组( P < 0.05), MAU、血清中CysC含量分别与冠脉计分呈正相关。结论MAU、血清中CysC含量的测定有助于判断高血压合并冠心病患者冠脉病变的严重程度及早期肾损害情况,从而指导临床治疗。高血压病患者中MAU、血清中CysC浓度持续显著升高可能是冠心病的一个危险和(或)加重因素。

    Abstract ObjectiveTo investigate the clinical implication of serum Cystatin C (CysC)and Microalbuminuria (MAU) levels in patients with hypertension and complicated with coronary heart disease (CHD ) . MethodsA total of 130 patients who were undergoing selective coronary angiography and who suffered from hype-

    rtension and suspected CHD, were enrolled in this study. The patients were classified into four groups, group CAD Ⅰ( single branch lesion) , group CAD Ⅱ ( coronary art-

    ery lesion vessels ≥ 2) , group group hypertensionand the healthy group. The imunity transmission turbidity and enzyme-linked immunosorbent assay methods were app-

    lied to measure the serum MAU and CysC levels. With healthy examiners as normal controls, the variations of respective concentration among four groups and their clinical implication were studied. Results The serum CysC and MAU levels of group CAD Ⅱ, CAD I, hypertension were significantly higher than the healthy ( P < 0. 05) , The serum CysC and MAU levels of group CAD Ⅱ was significantly higher than CAD I ( P < 0. 05) .The serum CysC and MAU levels of CAD have positive correlations among MAU and coronary heart disease ,ConclusionThe measurement of serum CysC and MAU is helpful to evaluate the severity of coronary disease and early renal injury of patients with hypertension complicated with CHD and can guide the corresponding treatment. The successive and significant elevation of serum CysC and MAU is probably a risk factor and ( or) a worse element of CHD among patients with hypertension ......
