社区高血压管理病人药物降压治疗管理及血压达标的影响因素探究(1)
【摘 要】目的：探讨参与某两个社区服务中心中接受治疗的高血压病人药物降压以及血压达标情况的影响因素。方法：对调查期间其两个社区共计637例高血压病人的病情、用药情况、日常行为及其恢复状况进行问卷调查。结果：调查表明社区内高血压病人的药物治疗率达98.9%；多种药物配合降压是大部分病人的才去的降压措施。血压控制达标率约为66.7%；不同年龄阶段、不同饮食习惯、是否吸烟或饮酒、治疗时长、血糖和血脂、是否产生并发症和是否遵从医嘱等病人的血压控制情况具有统计学意义(p<0.05)，在这当中，年龄高于六十岁、是否按时服药、并发症多少是血压达标的影响因素(p<0.05)结论：在社区卫生服务中心接受治疗的高血压病人绝大多数采用药物治疗的方法、遵循医嘱的病人血压达标状况比较好，另外血糖血脂状况影响病人的血压达标率。

Abstract Objective： To explore the influencing factors of drug hypotension and blood pressure compliance in hypertensive patients receiving treatment in two community service centers. Methods a total of 637 hypertensive patients in two communities during the survey were investigated by questionnaires， including their condition， medication status， daily behaviors and their recovery. Results the survey showed that the drug treatment rate of hypertension patients in the community reached 98.9%. The rate of blood pressure control was about 66.7%； the blood pressure control of patients in different age stages， different dietary habits， smoking or drinking， length of time， blood sugar and blood lipid， whether there were complications and compliance with medical advice， were statistically significant (p<0.05)， in which age was higher than sixty years， and it was on time. The incidence of blood pressure in the community health service center is the most important factor (p<0.05). Most of the hypertensive patients receiving treatment in the community health service center adopt the method of drug treatment， and the blood pressure of the patients following the doctor's orders is better. In addition， the blood glucose and blood lipid condition affects the rate of the patient's blood pressure.

高血压是困扰我国中老年人的心血管疾病之一，在整体人群，血压水平随年龄逐渐升高，目前已成为我国形势严峻的公共卫生问题。社区卫生服务中心自2009年以来按照国家规定接收并治疗高血压病人成为国家的基本公共卫生服务之一。随着各地基础卫生服务建设逐渐完善，社区出现高血压病人管理服务，更好的为高血压病人提供便利的治疗机会和贴近自身实际的治疗方案。提高社区高血压患者的治疗依从性，是提高降压达标率，防止心血管事件发生的有效手段[1]。而且随着人们生活质量的提高和不注意不良生活习惯等因素的影响使高血压发病率大幅上升并逐渐呈现出低龄化的趋势，因此研究高血压控制成功的案例与因素变得尤为重要，从而为社区高血压治疗提供合理有效方案。限于对社区高血压的研究和报道比较少，所以本研究采取问卷调查的方式更为实际具体的了解高血压病人的实际情况，并采取一定方法分析其影响因素并为社区高血压管理治疗提供借鉴意义。现报道如下。

1 资料与方法
1.1 一般资料
本研究针对本地某两个社区的高血压病人进行问卷调查。共计调查637例社区高血压患者，以中老年人为主，平均年龄在65岁左右。且患者均知情并同意本研究，一般资料具有可比性(P>0.05)，同时经过社区卫生服务中心伦理委员会批准。首先建立社区的健康档案，寻找愿意接受本次调查的病人，在规则上不邀请在家受照料行动不便的病人；不邀请有严重认知交流障碍的病人。
1.2 方法
横断面調查方法是本次调查的主要方法，调查问卷主要内容包括高血压病人的年龄阶段、是否有并发症、几种并发症、血脂是否达标和是否遵从医嘱服药等方面。
