尺骨鹰嘴骨折张力带固定研究进展(1)
【摘 要】背景：尺骨鹰嘴骨折发生率较高，其治疗方式包括保守治疗、外固定、内固定(张力带、钢板螺钉、空心钉、髓内钉)等，但简单移位的非粉碎性尺骨鹰嘴骨折，张力带固定仍然是应用最广的方法。目的：通过回顾尺骨鹰嘴骨折张力带固定方式及其进展，寻找其中可能存在的问题及需要进一步改进和研究的地方。方法：应用计算机网络检索中国知网、万方、维普、Pubmed、Medcine、Cochrane等数据库现有的所有文献，检索关键词为“尺骨鹰嘴骨折、张力带、olecranon fracture、tension band”，检索之后对文章进行分析。结果与结论：张力带固定简单移位的非粉碎性尺骨鹰嘴骨折，最早最普遍应用的方式为“张力带钢丝+克氏针”，后来不断对张力带、克氏针的材质、形状、固定方式进行改进，出现丝线、锚线、可吸收缝线、Cable-Pin、钢缆等张力带钢丝的改进替代产品；尾端带孔的克氏针、前端带螺纹的克氏针、三棱形的克氏针，以及金属空心螺钉、可吸收的螺钉等改进替代克氏针的产品；以及锚钉固定、张力带钢板固定等新型固定方式，不仅内固定稳定性更加可靠，而且术后并发症更少。

Abstract：BACKGROUND： The incidence of olecranon fractures is high，The therapeutic strategies include conservative treatment， external fixation， internal fixation (tension band， plate and screws， hollow nails， intramedullary nails etc). However， the tension band is still the most widely used method for simple dislocated and non-comminuted olecranon fractures.OBJECTIVE： To generally introduce the patterns and progress of tension band fixation in olecranon fractures.METHODS： A computer retrieval of CNKI，WAN-FANG，VPCS， PubMed， Medcine，Cochrane databases was performed with “olecranon fracture，tension band”as key words for relevant articles published so far. Initially， 175 articles were retrieved， and finally 120 articles were included in result analysis.RESULTS AND CONCLUSION： The earliest and most commonly used method for internal fixation of simple dislocated and non-comminuted olecranon fractures is "tension band and Kirschner wires".Continuous improvements in the aspects of Materials， shapes， fixtion patterns of tension band and Kirschner wires were performed， consequently ，silk sutures， Anchor Lines， absorbable seams， Cable-Pin， wire cable， etc， were produced as alternative products of tension band.And Kirschner wires with a hole at the end， a screw thread or a prismatoidal shape at the front， a metal hollow screw， an absorbable screw， etc， were produced as alternative products of general Kirschner wires. New-styled fixtion such as anchor fixtion and olecranon tension plating fixtion were created and exhibited more reliable stability， less complications.

尺骨鹰嘴骨折是肘部常见损伤，约占全身骨折的0.9%-1.6%，占上肢骨折的10%[1]，占前臂骨折的18%，平均发生率为11.5-12/10万*年[2]；受伤的平均年龄男性为50岁，女性为63岁。损伤机制为直接暴力或间接暴力，直接暴力又分为高能量损伤(例如足球运动、摩托车祸)、低能量损伤(例如站立高度摔倒，肘部着地，占70%[3])。直接暴力常导致鹰嘴横断或粉碎性骨折，大部分都会波及关节面；间接暴力多由肘关节屈曲位时附着于鹰嘴的肱三头肌猛烈收缩造成撕脱骨折[4]，多为横行或斜行骨折。, http://www.100md.com(汪志中)
