肾原发性类癌1例报道及文献复习(1)

【摘要】目的探讨肾原发性类癌的临床病理特征，提高对该肿瘤的诊断水平。方法对1例原发性肾类癌进行光镜、免疫组化观察，并结合文献讨论。结果患者女，47 岁。体检发现左肾占位4月。肿瘤边界清楚，以实性为主，部分囊性变，切面淡黄色。镜下：瘤组织呈梁索样排列，部分呈腺样、巢团状及菊形团状排列，浸润于增生的纤维结缔组织中，瘤细胞呈矮柱状或小圆形，胞浆中等，核大小形态一致，核分裂象很少见，未见坏死。瘤组织无包膜，侵及周围肾组织及纤维组织构成的囊壁。免疫组化示肿瘤细胞vimentin、CD34 和CD99 ( + ) ,bcl-2 部分( + ) ,desmin、SMA、MAS、HMB45、carponin、S-100 蛋白、CD1l7、CD31、FVIII、CK(AE1PAE3) 、EMA、Ki-67 和p53 均( - ) 。结论肾原发性类癌是一种非常罕见的肿瘤,诊断主要依靠病理形态学及免疫组化,并应与肾的其他梭形细胞肿瘤鉴别。

【Abstract】Objective To explore the clinicalpathological features of primary renal carcinoid tumor(PRCT) of the kidney. Methods A case of PRCT was examined with light microscopy and immunohistochemistry ,and related literature was reviewed. Results The patient was 47 years old woman. Clinical and radiological examination revealed a tumor in the renal pelvis. Grossly , the tumor was well-circumscribed with grey yellow color on cut surface. Histologically, the tumor was mainly composed oftightly packed cords and trabeculae arrangement,. imparting a ribbonlike appearance Cells are polygonal with granular eosinophilic cytoplasm and indistinguishable cytoplasmic boundaries. Nuclei are round to oval and uniform in size with rare mitotic events. Immunohistochemically , the tumor cells were strongly positive for vimentin、 S-100 were weak positive , but negative for CgA、EMA and TTF-1.Ki-67 index was less than 10%.Conclusions PRCT is a very rare renal neoplasm. Diagnonsis is based on its morphology and immunohistochemistry.
