自制导线针在腹腔镜小儿疝修补术中的应用研究(1)

【摘要】目的 通过与传统切开疝修补术比较，评价自制导线针在小儿腹股沟斜疝腹腔镜治疗中的疗效及并发症。方法 260名腹股沟斜疝的儿童分为两组：以2006年10月为界，之前入院的120例行传统切开疝修补术(对照组)，之后入院的140例应用自制导线针行腹腔镜下疝囊高位结扎术(腹腔镜组)，比较手术时间、术后伤口疼痛持续时间、术后并发症发生率(伤口出血、感染，尿潴留，皮下血肿等)、出院后随访复发率(30个月)。结果 应用自制导线针为140例患儿行腹腔镜小儿腹股沟斜疝疝囊高位结扎术，手术过程顺利，与传统手术相比，手术时间、住院时间、切口疼痛持续时间明显更短(P<0.01)、并发症发生率更低(P<0.05)、复发率和住院费用则无明显差异(P>0.05)。结论 应用自制导线针行腹腔镜下小儿疝囊高位结扎术，术后恢复快，切口小，损伤轻，手术时间及住院时间短，操作简单，安全，疗效可靠，值得临床推广应用。

The Application of the home-made wire needle in laparoscopic hernia plastic operation in children

【Abstract】Objective By comparison with traditional open hernia plastic operation,we have appraised the curative effect and complication of the home-made wire needle in laparoscopic hernia plastic operation in children. Methods 260 children who were attacked by oblique inguinal hernia are devided into two groups: traditional open hernia plastic operation group(120cases,before October 2006) and laparoscopic hernia plastic operation group(140cases,after October 2006).We have observed two groups’ operative time, duration of postoperative wound pain, incidence of postoperative complications (wound bleeding, infection, urinary retention, hematoma, etc.) and the recurrence rate after discharge(30months). ResultsIn comparison with traditional open hernia plastic operation, the operative time, hospitalization time and duration of postoperative wound pain of laparoscopic hernia plastic operation group was significantly shorter (P<0.01); incidence of postoperative complications was significantly lower (P<0.05); relapse rates and hospital costs was no significant difference (P> 0.05). Conclusion The Application of the home-made wire needle in laparoscopic hernia plastic operation in children may decurtate the operative time, hospitalization time and duration of postoperative wound pain and depress incidence of postoperative complications.

小儿腹腔镜疝修补术因其微创、术后恢复快、并发症少、疗效确切，特别是能一次性手术治疗隐匿疝等优势已经在临床上广泛开展。我院改进手术器械，应用自制导线针行腹腔镜下疝囊高位结扎术治疗小儿腹股沟斜疝，获得了满意疗效，现报告如下。
