肝细胞性肝癌AFP-mRNA检测外周血癌细胞的价值(1)

【摘要】目的 通过对原发性肝细胞癌(HCC)外周血中癌细胞的检测，为临床建立诊断肝癌转移的常规方法.方法 以甲胎蛋白(AFP) mRNA为HCC的标记物，应用nested RT-PCR对30例HCC外周静脉全血标本及不同细胞株进行检测及分析.结果 HCC患者30例外周血癌细胞阳性检出率为56.7%，并与肝内静脉癌栓形成(11/13，84.6% vs 6/17， 35.3%)、肝内转移(16/23，69.6% vs 1/7，14.3%)和细胞分化密切相关(P<0.05)；最低检出浓度为全血肝癌细胞2×104/ L. 对照组中结肠癌、鼻咽癌等肝转移癌以及胆囊结石患者的外周血中均未发现AFP mRNA. 结论 Nested RT-PCR对HCC的检测具有较强的特异性和较高的灵敏度，对于HCC诊断、判断转移可能具有较为重要的临床应用价值

HCC AFP-mRNA detection of peripheral blood cancer cells.

【Abstract】Objective Purpose through to the primary hepatocellular carcinoma (HCC) in peripheral blood for clinical testing of the cancer cells, a diagnosis of hepatic transfer routine methods. Method to a tire protein (AFP) mRNA for HCC marker of nested PCR, application of 30 cases of RT-HCC peripheral vein blood samples and the testing and analysis of different cell lines. Results patients with HCC 30 cases of peripheral blood cancer detection rate to 56.7%, and positive and liver inside vein tumor emboli form (11/13, 35.3%),84.6% vs 6/17, 16/23,69.6% within the liver metastasis (1/7) and < suried admission-free vs closely related to cell differentiation (P < 0.05); Minimum detection concentration for whole blood liver cancer cell 2 x 104 / l. in the control group, colon cancer, liver cancer and transfer such as nasopharyngeal carcinoma patients with cystic stone in the peripheral blood were not found mRNA AFP. Conclusion Nested RT-to the detection of PCR HCC has strong specificity and higher sensitivity, for HCC diagnosis, judge transfer may have an important value of clinical applications

目前临床上尚未建立一种能动态检测原发性肝细胞癌(HCC)尤其是手术后发生转移的常规方法. 从HCC患者外周静脉血中收集癌细胞并对其特异性标记物进行扩增将可能成为诊断HCC转移的重要手段之一. 我们拟采用AFPmRNA作为外周血HCC细胞的标记物，为建立监测HCC复发、转移作初步的探讨.
1 对象和方法
1.1 对象 西南医院1996-03/ 1996-11住院手术HCC 30例，对照组18例，其中肝转移癌8例，胆囊结石10例. 手术前3 d抽取外周全血，肝素抗凝，分离有核细胞，液氮冻存. HCC组：HCC细胞HpG2,PLC/ PRF；对照组：急性白血病细胞HL60，肺小细胞癌细胞KHC. 37℃，50 mL/ L CO2 ......
