【摘要】目的 观察甘露醇不同滴速对急性脑梗死患者血流变学及临床疗效的影响。方法 将100例无明显颅内压增高的急性脑梗死患者随机分为甘露醇快滴组(快滴组)和甘露醇慢滴组(慢滴组)。两组在常规治疗的基础上，快滴组予甘露醇150ml静滴，20分钟内滴完。慢滴组予甘露醇150ml静滴，60分钟内滴完。两组分别于治疗前，治疗后3h，6h进行血液流变学检查；在治疗前、治疗后7天、14天，用中国脑卒中病人临床神经功能缺损程度评分(CSS)标准评价临床疗效。结果 甘露醇静滴后3h两组血粘度均降低，6h快滴组测血粘度基本正常，而慢滴组6h所测全血粘度、血浆粘度均明显下降，较快滴组有显著差异(P<0.05)，慢滴组治疗后7天、14天CSS评分显著低于快滴组(P < 0.05)。秩和检验显示慢滴组疗效显著优于快滴组(P < 0.05)。结论 甘露醇慢滴改善血流变学指标更明显，降低CSS更显著，疗效优于快滴组。

Effects of different intravenous drip rate of mannitol on hemorheology and therapeutic effect in patients with acute cerebral infarction

【Abstract】Objective To observe the effects of different intravenous drip rate of mannitol on hemorheology and therapeutic effect in patients with acute cerebral infarction. Methods 100 patients, suffering acute cerebral infarction, without obviously intracranial hypertension, were randomly divided into two groups, one group of patients with rapid intravenous drip rate of mannitol, and the other with slow intravenous drip rate. Both of them received conventional therapy. In the first group, 150ml mannitol injection was done within 20min by intravenous drip ( ivdrip ), while the other one was done within 60min. Hemorheology parameters were tested before and 3h, then 6h after the treatment.We use China Stroke Scale(CSS) to evaluate the therapeutic effect before and 7d, then 14d after the treatment. Results Blood viscosity decreased in both groups 3h after the therapy; blood viscosity of the group of patients with rapid intravenous drip rate fell to the normal level 6h after therapy, while the group with slow intravenous drip rate had a obviously descent with whole blood viscosity and plasma viscosity(P<0.05).
