孕期热能个体化供给对子痫前期的影响(1)

【摘要】目的 探讨不同孕前体重指数(BMI)孕妇孕期理想的热能供给以降低子痫前期的发生。方法 随机选取健康的单胎初产妇2261例,1003例为研究组, 1258例为对照组,根据孕前或孕12周前BMI分组分别为正常体重组、超重组、肥胖组,并制定不同的热能供给方案,对照组热能供给不限制。指导孕妇进行轻体力劳动,系统观察胎儿增长规律、孕期体重、孕期体重指数增加幅度、子痫前期的发生及新生儿出生体重。结果 超重组和肥胖组中子痫前期发病率研究组与对照组有显著性差异(P <0.05),而在正常体重组中研究组与对照组各指标无统计学意义。影响子痫前期的因素经单因素筛选后进行多因素Logistic回归分析,表明:年龄、孕前体重、孕前体重指数、孕期体重增加、孕期体重指数增加、新生儿出生体重影响到子痫前期的发生。结论 对孕前不同BMI孕妇,给予不同的热能供给方案,可降低子痫前期的发生。

THE INFLUENCE OF HEAT SUPPLY FOR INDIVIDUAL DURING PREGNANCY ON PREECLAMPSIA

【Abstract】Objective To evaluate the supply of the ideal heat of pre-pregnancy Body mass index (BMI) during pregnancy to reduce the occurrence of preeclampsia. Methods Selecting 2,261 cases of singleton pregnancies which were divided into study group(1003 cases) and control group(1258 cases). According to the pregnancy or before 12 weeks pregnancy BMI group, the cases were divided into normal weight (BMI 18.5 ~ 22.9), overweight (BMI23.0 ~ 24.9 ), and obese (BMI ≥ 25.0), the study group and control group maternal age and BMI were not have significantly different. Research group developed a different energy supply options, according to the different BMI of the pregnancy or before 12 weeks pregnancy. The control group does not restrict the supply of heat. Guiding pregnant women have light work ,and each group observing fetal growth, pregnancy weight, pregnancy rate of increase in body mass index, incidence of preeclampsia and neonatal birth weight. Results There are significantly different between the study group and control group in overweight and obese groups(P <0.05), while the study group and control group in the normal weight group showed no statistical significance.After a single factor screening,we have Logistic regression multivariate analysis, show that: the incidence of preeclampsia was affected by age, pre-pregnancy weight, pre-pregnancy body mass index, pregnancy weight gain, increased body mass index during pregnancy, birth weight and other factors ......
