脑脊液置换加鞘内注药治疗结核性脑膜炎49例临床分析(1)

[摘要] 目的 分析脑脊液置换加鞘内注药治疗结核性脑膜炎的疗效,为临床应用提供依据。方法 以2004年1月至2009年1月在我中心收治的住院的49例结核性脑膜炎患者为观察组,采用脑脊液置换联合鞘内注药治疗结核性脑膜炎例,以我中心同期收治的43例为对照组,采用常规治疗。对两组治疗效果进行评估。结果 治疗组治愈率(87.76%)与对照组治愈率(67.44%)相比有显著性差异(P<0.05);治疗组脑脊液蛋白、压力、糖、氯化物、细胞数恢复正常时间均较对照组明显缩短(P<0.05)。结论 脑脊液置换联合鞘内注药可缓解结核性脑膜炎的症状,缩短疗程,该方法是治疗结核性脑膜炎的一种简单、有效、安全的方法。

Clinical analysis of Cerebrospinal Fluid Replacement Combined with Intrathecal Injection on Treating Tuberculous Meningitis on 49 cases
[Abstract]ObjectiveTo investigate the clinical effect of cerebrospinal fluld (CSF) replacement combined with intrathecal injection on treating tuberculous meningitis. MethodsA total of 49 cases of treatment group were treated with the method of CSF replacement combined with intrathecal injection and 43 cases of control group were treated with routine antituberculous drugs, hormone and dehydrant. The clinical effects were compared between the two groups, as well as the recovery time of CSF pressure, the levels of protein, glucose, chlorides, cells, and the time of hospitalization.ResultsThe cure rate was 87.76% and 67.44% respectively in the treatment group and in the control group. There was a significan difference between the two groups (P<0.05). The recovery time of CSF pressure was shorter in the treatment group than in the control group, as well as that of the levels of protein, glucose, chlorides and cells (P<0.05).ConclusionCSF replacement combined with intrathecal injection can relieve tuberculous meningitis' symptoms, and it is a simple, effective and secure method.

结核性脑膜炎(tubercolous meningitis)是临床常见的一种结核病类型,临床症状较为严重,致残率和致死率高,是近年来临床治疗中面临的难点之一[1,2]。2004年1月至2009年1月在笔者所在治疗中心采用脑脊液置换加鞘内注药治疗结核性脑膜炎49例,取得满意效果,现报告如下。
1资料与方法
1.1一般资料本研究收入的92例患者,均为我中心2004年1月至2009年1月收治的结核性脑膜炎患者,其中治疗组49例,男29例,女20例;年龄3~71岁,平均年龄(29.7±5.9)岁;病程7~35d,平均(21±4.1)d。对照组43例患者,男24例,女19例;年龄3~71岁,平均年龄(29.7±5.9)岁;病程8~36d,平均(19±3.9)d。
