[摘要] 目的 比较丙泊酚联合布托啡诺静脉麻醉及单用丙泊酚静脉麻醉在人工流产术中的镇痛效果和安全性。方法 将100例妊娠50～60d孕妇随机分为A组丙泊酚联合布托啡诺组50例，B组单用丙泊酚注射组50例，并对其术中镇痛效果、人流综合征发生率、术后宫缩疼痛，麻醉诱导时有无诉注射痛、术中有无肢动及术后有无诉腹痛尿急，清醒时间进行比较。结果 丙泊酚联合布托啡诺镇痛组总有效率、麻醉诱导时诉注射痛、术中肢动及术后人流综合征、术后宫缩疼痛发生率与单用丙泊酚组比较差异有统计学意义(P均<0.01)。结论 丙泊酚复合布托啡诺用于人工流产术可以减少丙泊酚用量，能有效抑制或减轻术后宫缩痛，降低人流综合征发生率，且不影响清醒质量。

[Abstract]Objective To compare propofol anesthesia and intravenous butorphanol alone propofol intravenous anesthesia in artificial abortion in the analgesic efficacy and safety.Methods 100 pregnant women from 50 to 60 days were randomly divided into propofol butorphanol group of 50 patients,a single injection with propofol 50 cases,and its analgesic effect,the incidence of abortion syndrome postoperative uterine pain, recovery time were compared.Results Propofol butorphanol analgesia group, the total effective complained of pain during induction of anesthesia, intraoperative and postoperative whether the body movement with or without abortion syndrome,the incidence of postoperative pain and uterine contractions propofol alone The difference between groups was significant (P<0.01).Conclusion The propofol butorphanol in induced abortion can reduce the total amount of propofol can inhibit or reduce pain after contraction,reducing the incidence of abortion syndrome,without affecting the quality conscious, butorphanol mild respiratory depression,but also inhibit postoperative nausea,vomiting and other adverse reactions.

丙泊酚由于起效迅速、作用时间短、凊醒完全而广泛应用于各种手术麻醉，特别是门诊短小手术的麻醉，但注射痛和缺乏镇痛作用是其缺憾之一，临床上常配与阿片受体阻滞剂(如芬太尼等)或局麻药(1%～2%利多卡因)预防注射痛或增强其镇痛效果。人工流产术是广泛应用于避孕失败的补救措施之一，但术中扩张子宫颈和刮吸子宫内膜时可使病人感到极度疼痛和不适，为解除受术者恐惧心理，减轻术中疼痛，减少人工流产中引起的综合反应及术后宫缩疼痛，本文着重观察了具有阿片类受体激动-拮抗双重作用的新药-酒石酸布托啡诺用于丙泊酚人工流产术时的镇痛效果，现将结果报道如下。
1 资料与方法
1.1 一般资料：选择2008年1月～2009年11月来我院门诊自愿要求行无痛人流术而无心血管疾病、生殖器官畸形及药物过敏史者及禁忌证的早孕妇女100例，年龄18～40岁，平均24.5岁，体重45～70kg，孕龄50～60d，超声显示为宫内妊娠，尿HCG阳性。随机分为A组采用丙泊酚联合布托啡诺静脉麻醉50例，B组单用丙泊酚静脉麻醉50例 ......
