Co2激光治疗结节性痒疹30例疗效观察

【摘要】目的:探索结节性痒疹的治疗方法及疗效。方法治疗组Co2 激光机治疗皮损平皮面或略凹皮面后，伤口外涂夫西地酸乳膏白天口服左西替利嗪胶囊5mgⅹ1粒，一天一次，一次一粒；晚上睡前多塞平片25 mgⅹ1片;对照组口服同治疗组, 皮损外涂复方氟米松软膏，一天二次，3周为一疗程并观察疗效，6个月观察复发。结果治疗组有效率为93.33%,痊愈率70.00%,对照组有效率为60.00%,痊愈率23.33%。两组有效率比较及两组痊愈率比较统计学均有非常显著性差异(p<0.01),治疗组显著优于对照组。结论Co2 激光机治疗结节性痒疹疗效显著可靠,值得临床推广。

Two group of effectiveness comparisons and two group of healing rate comparison statistics has the unusual significance difference (p<0.01), the treatment group obviously surpasses the control group. Conclusion Co2 stirs up the X-ray machine treatment nodular itching measles curative effect remarkable reliable, is worth the clinical promotion.

1 临床资料
2007年元月～2010年12月门诊收治的病例, 全部病例均符合赵辩主编的临床皮肤病学第3版中结节性痒疹的诊断标准，所有病例均排除瘢痕疙瘩体质，随机分为治疗组和对照组。治疗组30例中，男11例，女19例；年龄18-77岁，平均32.8岁；病程3个月-7年，年均11.5个月。对照组30例中，男9例，女21例；年龄16-75岁，平均33岁；病程4个月-8年，平均11.3个月。两组性别、年龄、病程经统计学分析，差异无统计学意义(P<0.05)，具有可比性。
2 治疗方法
治疗组：用可调Co2 激光机调其波长为10.6nm，输出功率8W~30W，光斑直径0.2mm，焦点四周的功率密度为254.8W/mm~955.4W/mm，刀头距离病灶0.5cm治疗皮损平皮面或略凹皮面后，伤口外涂夫西地酸乳膏;白天口服左西替利嗪胶囊5mgⅹ1粒，一天一次，一次一粒。晚上睡前服多塞平片25 mgⅹ1片。对照组：口服同治疗组, 皮损外涂复方氟米松软膏，一天二次，3周为一疗程并观察疗效，6个月观察复发。
3 疗效判定标准
痊愈为皮疹完全消退，痒感消失，仅遗留色素沉着或色素减退斑;显效为皮疹消退≥80%，痒感明显减轻;好转为结节缩小，皮损消退50%以上，痒感减轻;无效为皮损无改善或继续加重，痒感加重[1]。总有效率=痊愈率+显效率。
4 统计学处理
治疗组与对照组有效率比较采用χ2检验。
