新生儿吸痰流程改良后的临床观察

【中图分类号】R473.72【文献标识码】A【文章编号】1005-2720(2009)08-0284-02【摘要】目的：观察改良吸痰流程吸痰效果。方法：将60例需要吸痰的患儿随机分成两组，实验组采用改良吸痰流程吸痰，即采用10ml的生理盐水代替吸痰包；对照组采用传统吸痰流程吸痰，观察两组吸痰所用的时间及吸痰效果。结果：实验组吸痰所用时间明显少于对照组，且吸痰效果优于对照组。结论：改良吸痰流程能缩短操作时间，节省资源，效果也优于传统吸痰流程。

【Abstract】Objective：To observe the clinical effect of modified sputum suction flow-sheet for newborns accepted sputum suction.Methods：A tatal of 60 newborns who required sputum suction were randomly divided into test an control group(30 cases each).Newborns of control group were applied modified sputum suction flow-sheet，witch use 10ml of Physiological saline.Indexes including sputum suction time.The contamination of sputum tube and the effect of sputum suction of all cases were observed.Results：The sputum suction time of test group newborns was significantly less than that of control group cases.And the effect of sputum suction of the test group newborns was better than that of control group.Conclusion：Modified sputum suction flow-sheet was an effective way of sputum suction with shortened manipulation time.Yet，it can decnease the incidence rate of cross infection of patients.

在NICU这个特殊的科室，患儿娇小，病情多比较危急，而吸痰是清理呼吸道分泌物的有效措施，快速有效吸痰是呼吸道护理的关键，传统吸痰流程[1]存在备物耗时、流程繁琐问题，易延误抢救时机，为此对传统的吸痰流程进行了改进，效果满意。
1 资料与方法
1.1 临床资料：我科(NICU)自2008年6月~2008年12月共收治需要吸痰的早产儿60例，按照随机分组方法分为两组，每组30例。两组在性别、年龄、病情等方面差异无统计学意义(P>0.05)，具有可比性。
1.2 方法
1.2.1 研究方法：采用两组病人对比研究的方法，吸痰时机均按照适时吸痰来操作。
1.2.2 治疗方法：对照组采用传统的吸痰流程，备物：负压吸引装置、吸痰管、听诊器、手套、吸痰包、生理盐水、弯盘、治疗巾、手电筒、消佳净100ml，必要时备开口器、压舌板、舌钳[2]。吸痰包内有无菌容器、镊子，容器内装无菌生理盐水，用于吸痰前的试吸和吸痰中抽吸生理盐水冲洗，吸痰托盘要求4h更换1次。实验组吸痰时无需备吸痰包，只需手套，10ml的生理盐水和吸痰管。评估后，打开10ml的生理盐水，再戴手套，保持右手无菌，持吸痰管吸痰，左手拿连接管控制负压的有无，按吸痰常规操作吸痰，完毕，丢弃吸痰管及生理盐水的安瓿。相对传统流程的改良之处在于：①取消了无菌容器及大瓶的无菌生理盐水 ......
