关节镜清理术加透明质酸钠注射治疗膝关节骨性关节炎的临床研究(2)

透明质酸是构成关节软骨和滑液的主要成分，由关节滑膜的β细胞分泌，对关节的润滑、软骨的保护以及对滑膜液及软骨粘弹特性的维持均起着重要作用。研究表明，在OA患者的关节滑膜液中透明质酸的分子量和性能均有所降低[5]。关节腔内注射透明质酸后，可以在关节软骨面形成一层粘弹性保护膜，抑制滑膜细胞分泌尿激酶型纤溶酶原激活剂及其受体、基质金属蛋白酶和白介素等炎性介质[6~8]进而抑制T细胞和B细胞的活化，减轻免疫损害。
本研究结果提示骨性关节炎术后6个月，关节镜清理术配合透明质酸钠注射治疗的疗效优良率高于单纯的关节镜清理术，且统计学上有显著差异；由于远期失访率较高，本组没有晚期疗效进行观察对比。
