摘 要 回顾近10年来我国慢性病防治工作的现状，借鉴国外慢性病管理的成功经验，对慢性病管理模型的基本元素和评价方法进行了介绍，以供社区慢性病管理者参考。

    ABSTRACT The basic element and evaluation method of the chronic disease management model are introduced by reviewing the current situation of the chronic disease prevention and treatment in our country and using the successful experience of foreign chronic disease management for reference so as to provide a reference for the community chronic disease managers.
