摘 要 本文对上海市徐汇区试行社区慢性病长处方的做法进行了介绍，徐汇区为了实施社区慢性病长处方，率先突破了门诊均次费用评价、药占比、基本药物制度目录等限制，在慢性病分治、分级诊疗方面进行了有意义的探索。
